About this role

Job summary

An exciting secondment is currently available for the role of a Social Prescriber within Greengates Primary Care Network. The secondment will involve being part of the YOURhealth team within Humber Teaching NHS Foundation Trust covering the patient cohort within Greengates PCN with the aim of improving the health and well-being of the local community.

The role involves supporting patients across the Greengates Primary Care Network across Cottingham and Beverley. You will offer individualised, personal support and motivation to adults who may need solutions to social, emotional, or practical needs. This support will help empower each individual accessing the service to empower themselves, leading to improved health and wellbeing.

You will work with our existing team of dedicated Social Prescribers as well as our Mental Health and Wellbeing Coaches and Health and Wellbeing Coaches.

Main duties of the job

You will also be expected to promote and market healthy lifestyle issues, including social health within the local communities to encourage change within areas of deprivation and isolation.Excellent interpersonal communication skills are an essential requirement of Social Prescribing as you will be working with individuals (and potentially groups of people) from a variety of backgrounds and have a sound understanding of behaviour change techniques and asset based community development.

Although the role is currently Monday to Friday during office hours there may be rare occasions when you will be required to work evenings and weekends. You will also be required to commute effectively across the geographical boundaries of Hull and East Yorkshire for clinics, meetings and training.

Person Specification

Qualifications, Education and Training Essential

Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving Ability to travel between sites in a timely manner

Knowledge and Experience Essential

Excellent group and one-to-one facilitation skills including conflict resolution Proficient in Microsoft Office and web-based services

Desirable

Level 3 Social Prescribing qualification

Skills, Competencies and Personal Qualities Essential

An understanding of the biopsychosocial model of health and the social determinants of health Knowledge/experience of prevention and mental health

Desirable

Coaching/counselling qualification/ experience or other relevant qualification/experience involving reflective listening skills.
