Job summary
Worthing Medical Group is a large, forward-thinking primary care practice delivering a well-established, highly proactive frailty service across care homes and for housebound patients. We currently support over 900 patients living in care homes, with a whole-system model designed to deliver high-quality, coordinated care to one of our most vulnerable patient groups.
The service is clinically led by GP Lead Dr Anna Robertson, working alongside a team of three highly skilled Advanced Nurse Practitioners, supported by dedicated care coordinators. This creates a strong, collaborative and well-supported environment, enabling clinicians to focus on delivering high-quality care.
We operate a structured, proactive approach including care home ward rounds, case management and multidisciplinary working, ensuring patients receive the right care at the right time. Clinical support is a key strength of the service, with a dedicated weekly GP supervision session for complex case discussion, alongside access to GP support throughout the working day for advice and escalation.

JOB SUMMARY:

KEY responsibilities:
Undertake regular ward rounds within care home settings
Provide clinical assessment and management of frail and complex patients
Deliver proactive case management for housebound patients
Work collaboratively within a multidisciplinary team (MDT)
Coordinate care with external services to ensure a joined-up approach
Develop personalised and anticipatory care plans
Support end-of-life care planning
Identify deterioration early and escalate appropriately
Ensure accurate documentation and coding of clinical activity

Qualifications Essential
Registered Nurse (NMC) or Paramedic (HCPC) or relevant AHP registration
Desirable
Advanced Clinical Practice qualification or equivalent experience
Experience Essential
Registered Nurse (NMC) or Paramedic (HCPC) or relevant AHP registration Experience working with frail, elderly or housebound patients Experience working with patients with dementia or cognitive impairment Strong clinical assessment and decision-making skills Experience in community, primary care or care home settings Ability to work independently and manage a clinical caseload Experience working within multidisciplinary teams
