Job summary
An exciting opportunity has arisen to join the Community Enablement and Recovery Team (CERT) within the Black country. CERT work with people with a severe and enduring mental illness who require community rehabilitation support.
As an Occupational Therapy Assistant Practitioner [OTAP], you will be supporting the CERT OT provision in providing tailored community OT rehabilitation in the persons home and within their local community. You will be working in collaboration with service users, family/carer and other professionals,providing holistic,person-centred, evidence-based,andtrauma-informedcarewith a proactive approach and ability to providehighintensive interventions.
We aim to providerecovery orientated rehabilitation serviceswith the aim tocollaborate with andempower individuals withmental healthrehabilitation needsacross the Black Country to achievean optimumquality of life.
Main duties of the job
The Occupational Therapy Assistant Practitioner will work within defined competencies and without direct supervision to provide a flexible range of therapeutic activities for adults within their own homes and local community.
Under supervision and guidance of the OT, the post holder will take responsibility for supporting a pre directed care plan for the service user, contributing to the implementation of clinical care. The post holder will also support the use of MOHO based assessment tools during their practice and be able to feedback to the OT to inform formulation.
Undertaking individual therapeutic activities with clients to promote independence and wellbeing. Assessing client's responses to activities and amending programmes of activity where necessary, making judgements on when to escalate clinical issues to the OT.
To work collaboratively with MDT members/service providers in the provision of supporting the service user. To effectively communicate and highlight any clinical risks and report this to the MDT in a proficient and timely fashion and if necessary, contribute to ongoing risk reduction and management.
To gather information from the service user/carer as part of the care plan process and activities identified within it, communicating effectively with clients where there are barriers to understanding and where clients are highly emotive or distresses and often presenting challenging behaviour.

Some travel will be required to visit out of area placements to support assessing service users.
The service will operate Monday to Friday 9-5 hours but is expected to advise service users and carers of access to services outside of these hours and liaise and refer on to other mental health care providers in cases where support outside of these hours is required.

Education and Qualifications Essential
Relevant Level 4 or above qualifications - i.e.: Foundation Degree, NVQ level 4 or equivalent

Evidence of training and development within OT Has evidence of completing the care certificate or willing to work towards.
Knowledge and experience Essential
