Job summary

Owing to a new service development, there is an exciting opportunity for a wellbeing practitioner/ social prescriber to join a multi-agency team caring for young people 16+ in a supported accommodation setting.

Nottinghamshire County Council (NCC) Children and Families Commissioning and the Contracts team in collaboration with Nottinghamshire Healthcare foundation trust and Change Grow Live, intend to build on the existing 16+ Supported Accommodation model by integrating a multi-disciplinary team (MDT) to deliver a specialist psychologically informed service to young people in the Supported Accommodation model.

The need to do something different arose from the strategic review of 16+ Supported Accommodation provision where it was identified that young people and commissioned providers would benefit from access to specialist mental health/wellbeing and substance use specialists. Nottinghamshire County Council made a successful bid for DfE funding to provide a Staying Close Offer in Nottinghamshire. A key component of the bid was the creation of the Multi-Disciplinary Team( MDT).

Research and evidence has shown that a high proportion of young people in supported accommodation have experienced significant childhood trauma and that past trauma affects young people in the present day. This can lead to challenging behaviour in supported accommodation services which may contribute to breakdowns in accommodation placements.

Main duties of the job

The MDT will consist of mental health practitioners, wellbeing workers and substance use workers. The team will provide support and interventions to prevent young people feeling overwhelmed with the day-to-day challenges of managing their wellbeing, childhood trauma, or self-medication with substances. Young people will have better access to specialist services, will feel better able to manage their emotional wellbeing, mental health and reduce any harmful substance use.

The MDT will also support the development of the skills and emotional resilience of the workforce- supporting with vicarious trauma in the form of psychologically informed incident debrief and reflective practice.

The successful candidate will be required to work form Sherwood Energy Village, Ollerton on Mondays. Young people who are supported by the team will live in accommodation services, commissioned by Nottingham County council in Newark and Bassetlaw.

Job description Job responsibilities

The post holder will work as part of a small multi-disciplinary team, delivering high-quality; brief outcome focused evidence-based interventions for young people experiencing mild to moderate mental health difficulties including anxiety, low mood and behavioural difficulties within a local authority placement setting and will be required to support young people in accessing other appropriate community support and connecting them to activities, groups and services in their community to meet their practical, social and emotional needs to support positive emotional wellbeing.

Provide personalised support to young people to enable them to take control of their wellbeing, live independently and improve their health outcomes.

To work within the trust framework of the Looked after Childrens team within specialist community CAMHS and to work into the specified local authority placements.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.You are encouraged to enrol for the DBS Update Service.
