We aWe are looking for an experienced and enthusiastic Advanced Nurse Practitioner to join our friendly and supportive primary care team.

This is an exciting opportunity for an ANP who is passionate about providing high-quality, patient-centred care and enjoys working as part of a wider multidisciplinary team.

The successful candidate will provide autonomous assessment, diagnosis and management of patients with a range of acute and long-term conditions. You will use your advanced clinical skills to support patients, improve access to care and work alongside our GPs, nurses and wider practice team.

Main duties of the job

Brief description of main duties The Advanced Nurse Practitioner will work as part of the practice team to provide safe, effective and high-quality care to patients.

Main duties will include:

Assessing, diagnosing and treating patients with a range of acute and long-term conditions.

Carrying out clinical assessments and making appropriate treatment decisions.

Prescribing and managing medicines within their scope of practice.

Supporting patients with ongoing health needs and care plans.

Providing health advice, preventative care and health promotion.

Working closely with GPs, nurses and the wider healthcare team to provide joined-up patient care.

Making referrals where appropriate and following clinical pathways.

Keeping accurate patient records and ensuring clinical documentation is completed.

Taking part in audits, service improvements and practice meetings.

Supporting the learning and development of colleagues where appropriate.

Keeping up to date with clinical guidance, training and professional requirements.

The post holder will play an important role in improving patient access, supporting the clinical team and delivering excellent care to our patients.

Job description Job responsibilities

The Advanced Nurse Practitioner (ANP) will work as an autonomous practitioner within Hawkinge & Elham Valley Practice, providing high-quality clinical care to our patient population.

The post holder will assess, diagnose and manage patients presenting with a wide range of acute and long-term conditions, using advanced clinical skills and professional judgement. The ANP will work collaboratively with GPs, nurses and the wider multidisciplinary team to improve patient access, outcomes and experience.

Key ResponsibilitiesClinical Care

Work autonomously to assess, diagnose and treat patients within your scope of practice.

Manage a range of acute presentations and long-term conditions.

Undertake clinical assessments, examinations and reviews.

Develop and implement appropriate treatment plans.

Prescribe medications safely in line with current legislation and professional guidance.

Provide advice on health promotion, prevention and self-management.

Recognise and manage patients who are deteriorating or require urgent intervention.

Make appropriate referrals and follow agreed clinical pathways.

Provide appropriate follow-up and continuity of care where required.

Support the management of patients with complex health needs.

Patient Care and Communication

Provide compassionate, person-centred care that respects patients needs and preferences.

Communicate effectively with patients, relatives and carers.

Support patients to make informed decisions about their health.

Ensure patients receive appropriate information, advice and support.

Maintain confidentiality at all times in line with professional and organisational requirements.

Team Working

Work collaboratively with GPs, nurses, healthcare professionals and the wider practice team.

Participate in multidisciplinary team meetings and clinical discussions.

Provide support and advice to colleagues where appropriate.

Contribute to the development and improvement of practice services.

Support a positive team culture focused on quality, safety and continuous improvement.

Clinical Governance and Quality Improvement

Maintain accurate and timely clinical records.

Participate in clinical audits, significant event reviews and quality improvement activities.

Follow practice policies, procedures and clinical guidelines.

Support the delivery of safe and effective services in line with CQC requirements.

Identify areas for improvement and contribute to service development.

Maintain professional registration and undertake relevant training and development.

Training and DevelopmentAs a training practice, the post holder will be expected to support a learning culture within the practice by:

Sharing knowledge and skills with colleagues where appropriate.

Supporting learners and trainees within the clinical environment.

Participating in teaching and supervision activities where appropriate.

Keeping up to date with developments in primary care and advanced clinical practice.

Person Specification

Knowledge and Skills Essential

Excellent clinical assessment, diagnostic and decision-making skills. Ability to work independently while recognising when support or referral is required. Excellent communication and interpersonal skills. Commitment to delivering safe, effective and patient-centred care. Ability to manage a varied and complex workload. Understanding of confidentiality, safeguarding and professional responsibilities. Ability to work effectively as part of a wider practice team. Commitment to continuous learning and professional development. Commitment to equality, diversity and inclusion.

Desirable

Understanding of NHS primary care and current healthcare challenges. Knowledge of CQC standards and clinical governance requirements. Interest in supporting learners within a training practice environment. Flexible approach to service development and new ways of working.

Experience Essential

Significant post-registration clinical experience. Experience of working as an autonomous practitioner. Experience assessing, diagnosing and managing patients with a range of acute and long-term conditions. Experience of prescribing and managing medicines within scope of practice. Experience working collaboratively within a multidisciplinary healthcare team. Experience of managing clinical risk and making appropriate clinical decisions.

Desirable

Experience working within general practice or primary care. Experience of supporting and developing other healthcare professionals. Experience of contributing to clinical audits, quality improvement projects or service development. Experience working within a training or teaching environment.

Qualifications Essential

Registered Nurse with current NMC registration. Advanced Nurse Practitioner qualification or equivalent advanced clinical practice qualification. Independent prescribing qualification. Evidence of ongoing professional development and maintenance of clinical competence.

Desirable

Further postgraduate qualifications relevant to advanced practice. Relevant training in areas such as minor illness, long-term conditions, clinical assessment or diagnostics. Evidence of teaching or supervision qualifications.
