Job summary
We currently have a fixed term vacancy available for a Community Learning Disabilities Nurse to work within the Sefton community team, supporting adults who have a diagnosis of a learning disability with complex physical and mental health diagnosis. Work as part of a dynamic supportive MDT. Plus the participation in an on call rota.
The post holder will manage their own caseload with the support from the wider MDT.
Main duties of the job
Supporting individuals with a diagnosis of a learning disability with mental and or physical health issues. Maintaining a caseload with support from your supervisor and team. As a team we support individuals with a learning disability with many aspects of their life such as supporting with acute physical health issues, groups and also through a variety of pathways both within the trust and external.

Person Specification
Qualifications Essential
Registered nurse qualification - Learning Disabilities RNLD Current NMC Licence to Practice Portfolio demonstrating evidence of learning and development
Desirable
Training qualification ENB 998 or equivalent
Experience Essential
Demonstrate experience at pre registration level of working across a wide range of clinical areas Experience of nursing care planning process for adults with Learning disabilities
Knowledge Essential
Basic knowledge of national and local policy / good practice guidance relating to care of people with learning disabilities Awareness of specialist assessment tools
Skills Essential
Ability to communicate effectively at all levels with other professionals and with people who may have significant barriers to understanding. Demonstrates good standard of verbal and written communication skills including basis I.T. skills Ability to work effectively within a multidisciplinary team Able to plan and manage own workload Ability to work in an unpredictable work environment which may include people with challenging and aggressive behaviour Flexible approach to hours and duties Keen to progress own personal development Must have the ability to travel within the area of work
