Job summary
Caterham Valley Medical Practice, part of The House Partnership, are looking for an enthusiastic, motivated Nurse to join our forward thinking practice.
Main duties of the job
The successful candidate will work collaboratively with our other Practice Nurses and HCAs to treat our patients, maintain systems including infection control to meet CQC requirements and influence and implement policy and change in the Group.
You can be newly qualified or a seasoned professional - either way, training will be given to integrate you into our team.
The position is part time for a minimum of 16 hours per week but more may be offered to the right candidate.Salary is competitive, depending on experience, with access to NHS pension scheme.

Job description Job responsibilities
The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
To act as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance and the annual statement.
To develop, implement and embed health promotion by participating in national screening and well-being programmes including ECG monitoring, BPs, cervical cytology etc
To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health
To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF including ear syringing, venepuncture
To provide wound care (ulcer/Doppler etc.) to patients
To provide travel medicine services
To request pathology services as necessary
To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
To maintain accurate clinical records in conjunction with extant legislation
To ensure read codes are used effectively
To maintain chronic disease registers
To chaperone patients where necessary
To assist GPs with minor surgery when required
To prioritise health issues and intervene appropriately giving advice on health, lifestyle, diet, smoking, alcohol intake, sexual health and contraception
To support the team in dealing with clinical emergencies
To recognise, assess and refer patients presenting with mental health needs
To implement vaccination programmes for adults and children
To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
To support the clinical team with all safeguarding matters in accordance with local and national policies
To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
To deliver opportunistic health promotion where appropriate
To collect data and participate in regular Practice audit and research projects
To contribute to the preparation of any Practice development plans
To maintain own annual registration with the NMC and adhere to Code of Professional Conduct
To maintain a safe and therapeutic environment for all staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to
To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice.
Attend and participate in staff and Practice meetings and in-house clinical training sessions where necessary
