Job summary
We are seeking a motivated and compassionate Band 5 Community Psychiatric Nurse to join our established Locality Mental Health Team in Skegness, providing high-quality, recovery-focused care to adults with a range of complex mental health needs.
This is an excellent opportunity for a newly qualified or early-career mental health nurse who is passionate about community practice and eager to develop clinical skills. You will work as part of a supportive multidisciplinary team, delivering evidence-based interventions, contributing to holistic assessments, and supporting safe and effective care planning.
We are looking for someone who is passionate about community mental health and recovery-focused practice and able to build therapeutic relationships with service users and carers.
You will be a reflective practitioner committed to learning and professional growth and a team player with strong communication and organisational skills.
As a Band 5 nurse, you will receive structured supervision, preceptorship, and development opportunities to build confidence in autonomous practice, risk management, and care coordination.
Main duties of the job
Comprehensive assessment -- Undertake biopsychosocial assessments, formulation, and risk assessments for adults referred to the service.
Case management -- Manage an allocated caseload, developing person-centred care plans in line with the LMHT SOP and Framework.
Risk management -- Contribute to dynamic risk assessment, crisis planning, and safety interventions.
Therapeutic interventions -- Deliver evidence-based interventions, including psychological, psychosocial, and recovery-focused approaches.
Duty work -- Participate in the team's duty rota, responding to urgent referrals and supporting crisis presentations.
Medication Management - Administer depot medications in the community and support service users prescribed clozapine medication in line with policy and guidance and with support of MDT.
Multidisciplinary collaboration -- Work closely with psychiatry, psychology, social care, primary care, and voluntary sector partners.
Clinical documentation -- Maintain accurate, timely, and professional records in line with Trust and NMC/HCPC standards.
Service development -- Contribute to quality improvement, transformation initiatives, and trauma-informed, recovery-oriented practice.
Supervision and support -- Provide supervision, mentorship, and support to junior staff and students.
Safeguarding -- Uphold safeguarding responsibilities for adults and children, working within statutory frameworks.
