The Clinic Nurse is a key member within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver and evaluate care, communicate findings, influence and drive change and promote health and best practice, patient safety and experience within the Clinics.
We are looking for an experienced registered nurse or a new qualified nurse who, acting within their professional boundaries, provides leadership and ensures the clinical governance framework standards within the clinics are upheld.
You will provide care for the presenting patient within the Clinics from initial clinical assessment, administration of prescribed medication, monitoring of Health and well-being of the Patient, including physical health interventions and evaluation of care.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
The post will also include supporting patients at home with their mental and physical health needs.
Main duties of the job
Work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.Be able to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process.To Administer prescribed depot/LAI antipsychotic medications in accordance with Trust policies, local protocols, and NMC guidelines.Carry out pre-injection assessments including mental state examination, physical health monitoring (e.g. blood pressure, pulse, weight, blood tests as required), and side-effect screening (e.g. EPSEs, metabolic syndrome, QTc risk).Carry out annual physical health checks as per the NICE guidelines.Assess and document the service user's response to medication and report concerns to the prescribing clinician.To take blood samples from patients and ensure that blood samples are analysed using the PocHi and where required sent to the laboratory within specified time scales.Managing blood results and take actions needed when results are outside normal parameters, following policy and national guidance

While prior experience in clozapine and depot monitoring is desirable, we recognise that this is a specialist area. We are committed to supporting staff to develop into the role, including providing training and supervision once a foundational understanding of the core principles of clozapine and depot monitoring is established.Monitor service users for adverse drug reactions and act appropriately, including escalating any concerns.Monitor, evaluate and modify therapeutic interventions in order to measure progress and increase efficacy for the best possible service user outcomes and experience

Previous experience of working in a community team & crisis resolution Some experience of managing a busy caseload of individuals with dual diagnosis. Experience in implementing new service delivery/change management

Experience in managing Change within a clinical setting.. Previous experience of recovery focused service provision with individuals who have complex mental health needs, substance misuse and offending behaviour.
Knowledge/ Skills Essential
Leadership skills including negotiating and influencing skills Knowledge and understanding of Adult safeguarding Recovery Model
Desirable
Understanding of the requirements of Quality Improvement Methodology including QSIR . Understanding of cardio metabolic syndrome and physical health in mental health.
