Job summary
The Social Prescriber will support individuals to improve their health and wellbeing by connecting them to non-medical community services. Working as part of a multidisciplinary team within primary care networks, they help people address social, emotional, and practical needs that impact their health, such as loneliness, housing, employment, or financial challenges.
Main duties of the job
Provide person-centred support through one-to-one consultations
Assess individuals needs strengths, and goals using a holistic approach
Develop personalised care and support plans
Link people to appropriate community, voluntary, and statutory services
Build strong relationships with local organisations and community groups
Support individuals to manage long-term conditions and improve wellbeing
Empower clients to build confidence resilience and independence
Maintain accurate confidential records of interactions and outcomes
Work collaboratively with GPs nurses and other healthcare professionals
Safeguard vulnerable adults and follow relevant policies and procedures

Person Specification
Experience Essential
GCSE A to C or equivalent in Maths and English Excellent communication skills (written and oral) Strong IT skills Clear polite telephone manner Effective time management (planning and organising) Ability to work as a team member and autonomously Problem solving and analytical skills Flexibility to work outside core office hours Driving licence and access to a vehicle
Desirable
Experience of working in a primary care environment. SystmOne user skills
