Job summary
The post holder is responsible for coding and recording clinical information from hospital correspondence and other sources into patient records in line with practice policies.
The role includes monitoring the generic email account, maintaining the palliative care register, arranging care plan review appointments, and supporting palliative care meeting preparation.
The post holder will maintain strict data security and confidentiality standards and actively engage in ongoing training and development.
Main duties of the job
Accurately process, scan, index, code, and summarise patient records and clinical correspondence, ensuring data is entered into the correct patient record.Manage incoming correspondence, electronic discharges, referrals, admissions, and safeguarding-related communications, escalating actions where required.Maintain patient records, including palliative care registers, ReSPECT/Care plans, deceased records, and related administrative processes.Monitor clinical systems, shared inboxes, portals, and administrative tasks to ensure timely action and adherence to protocols.Uphold patient confidentiality, maintain quality standards and productivity targets, and actively contribute to team training, meetings, and continuous development.Coordinate recalls, screenings, NHS health checks, immunisations, and follow-up appointments in line with practice procedures.Monitor and action clinical administration tasks, including pathology results, triage requests, screening outcomes, and patient record updates.Liaise with patients and colleagues to obtain information, resolve queries, arrange appointments, and support accurate clinical coding.Contribute to team development through training, mentoring new staff, supporting inductions and participating in health awareness initatives.

Qualified to GCSE standard or NVQ equivalent

Desirable
