Job summary
We are looking for enthusiastic and motivated individuals with experience in health and social care to join our highly regarded Urgent Community Response (UCR) Service, based at Woking Community Hospital. We provide a 24-hour, 7 Day urgent responsive NHS community services to patients across Northwest Surrey. The Out of Hours UCR Team work overnight forming part of the 24-hour UCR service and there is flexibility within the UCR team to work across days, nights and to do internal rotation.
Working as a member of the UCR team you will work with patients within an agreed level of competence, who may require management and/or rehabilitation interventions in their own environment to prevent acute hospital admissions and to support complex hospital discharges.
Although expected to complete home visits independently, you will not be alone. You will be working under the guidance and supervision of a qualified health professional i.e. Nurse, Occupational Therapist, Physiotherapist, to carry out the intervention.

Main duties of the job
To work as a Senior Community Rehabilitation Assistant, as part of the home-based Urgent Community Response (UCR) service, aiding rehabilitation and preventing hospital admissions. To work both independently and as part of a team managing a delegated caseload, under the guidance and supervision of a qualified professional e.g. Physiotherapist, Occupational Therapist, Nurse. To assist the UCR coordinators in maintaining the smooth running of the service and to work towards achieving a high quality and effective Urgent Community Response service, by being a proactive and integrated member of the team. Providing a seven-day service within recognised shift patterns which can include being part of the Day service or part the Out of Hours (OOH) service as UCR provides support over 24 hours, 7 days a week.

Communication / Relationship Skills
Following appropriate induction, supervision and competency-based training; you will be able to work alone with patients, providing prescribed rehabilitation programmes of care.
Have a thorough understanding of all programmes of care and rehabilitation within the UCR Service.
To provide therapeutic and social support as stated in patients individual intervention plans (which are set by a health or social care professional) whilst encouraging the patient to remain as independent as possible.
To assist the trained staff / coordinators in daily allocation of Senior Community Rehabilitation Assistants case load.
Work as part of the multi-disciplinary team (MDT) to provide crisis intervention, palliative care and any other components of the UCR service (and others as they are developed).
Responsible for informing, discussing and changing visits with clients as agreed by the MDT.
To work with qualified members of the MDT in setting patient specific goals and assist with monitoring the outcomes of therapeutic interventions.
To complete patient electronic records and patient held notes with up-to-date information.
To effectively communicate changes in condition or interventions with all members of the MDT and other agencies involved.
Respond to requests from GPs and Community Nurses for UCR intervention for patients with acute and palliative care needs, under the supervision of the on-call coordinator or the Service Operational Lead.
Participate in implementing initial care planning, and allocation of visits within available resources, following communication and discussion with relevant professional / coordinator.
To receive calls from colleagues on a late shift to ensure they are working safely and in compliance with the services lone working policy.
To liaise with the Operational Lead or OOH Clinical Coordinator during shifts, for support and advice or to provide any relevant updates and changes that may affect the service.
Responsible for contacting clients if delayed for any unforeseeable reason and for informing the office.
To communicate effectively appropriate information in an understandable format to patients, carers and other staff.
Understand patients right to choose, privacy and dignity and the need to treat patients as individuals.
Responsible for reading and adhering to patients intervention plan prior to commencing activities
Responsible for time management and spending appropriate time on each visit, ensuring the patient does not feel rushed or uncomfortable.
To use effective communication skills to gain co-operation with treatment where there may be significant barriers to understanding such as sight, hearing, cognitive or language barriers.
Responsible for the delivery of care and rehabilitation without prejudice maintaining a professional manner in sometimes adverse conditions in patients home environment.
Responsible for maintaining own competencies in all matters of patient care and rehabilitation as appropriate. For example, personal care, foot care, mouth care, tissue viability, catheter care, nutrition and fluid intake, infection control, supervision of taking medications, the use of personal protective equipment, waste disposal, health and safety, safe usage of equipment, patient transfers, correct walking gait, cognitive and functional activities. Inform line manager or mentor immediately if you do not feel competent to carry out task or care.
Can demonstrate an awareness and understanding of individuals physical conditions / symptoms, emotional, psychological, social, spiritual and cultural needs.
Have an understanding of the additional services available in the community and when and how to refer to these services, for example Meals on Wheels, Red Cross, Citizen Advice Bureau, Age UK and other voluntary services.
Can identify changes in patients condition and, within expected level of competence, act accordingly and in a timely fashion. If appropriate seeks advice from Operational Lead/Clinical Coordinator, or other professional before calling emergency services.
Following training and deemed competent, be able to supply and fit any aids and equipment (e.g. walking aids, chair raisers and commodes) as required, ensuring appropriate sizing and education of patients / carers in correct use of appliance/s. Reports defects and unforeseen risks regarding equipment to appropriate member of the MDT.
Responsibility for monitoring peripheral equipment store and ensuring correct procedure is followed when equipment is delivered to or removed from the store.
To collect and deliver equipment for clients in line with safe working practices, health and safety and infection control policy and procedures.
Understands the need for confidentiality and the Data Protection Act when recording patients personal and sensitive information. Maintains accurate, comprehensive, legally compliant and up-to-date patient notes in line with professional standards and local trust policies.
To adhere to set clinical protocols as required.
Aware and responsible in helping to maintain a safe working environment within the UCR office. Has a working knowledge of the organisations Health and Safety policy.
Responsible for representing the service in all activities, including introducing visitors to the service and ensuring patients receive the highest standard of care within given resources.
Under the guidance of the UCR Team Leaders is responsible for the smooth transition of care to other agencies, ensuring an up-to-date account of day-to-day care is given to the incoming agency. This may on occasion require a meeting with the patient / relative / carer and receiving agency.
Analytical / Judgemental Skills
To be aware of possible risks in patients home environment prior to undertaking individual patient care plans as directed by the team professionals.
Report any identified risks to the appropriate professional team member and be responsible for altering the patients care plan following advice and instruction from that professional.
To provide personal, therapeutic and social support as stated in patients individual care plans encouraging the patient to be as independent as possible.
Able to work independently in the community and UCR office whilst understanding own competency levels and limitations. Knowing when to seek advice and assistance from UCR Team Leaders.
Understand the need to respond to physical symptoms, emotional, psychological, social, spiritual and cultural needs of the patient, knowing own limitations and seeking advice from the appropriate professional when required.
Able to identify changes in patients condition and within level of competence act accordingly, asking for support and advice from the Operational Lead/Clinical Coordinator as required. Contacts emergency services directly when appropriate to do so.
Has good understanding regarding timings of visits, taking into account travelling and weather conditions and time required to record activities in patient notes.
Participate in personal and performance review and to undertake identified training needs and development. Demonstrate on-going continuing professional development.
Undertake clinical supervision on a regular basis.
To attend mandatory / statutory / in-service training and any other training programmes as identified in your personal development plan. To participate in the CSH Surrey appraisal scheme.
