Job summary
Join and lead our well established and highly skilled practice employed pharmacist team. This is a permanent post, working solely for Air Balloon Surgery, and funded through the FABB PCN Additional Roles Reimbursement Scheme.
The post has become vacant as the existing post holder is leaving to pursue a new venture outside General Practice. We have a long tradition of employing our own Practice Clinical Pharmacist, being one of the first in Bristol to do so. We fully recognise the value and important role this brings to the wider team. The pharmacy teams work is well established into practice pathways and working arrangements.
The role of the Senior Clinical Practice Pharmacist is well embedded as a senior clinical role within the wider practice clinical team. Leading our existing Pharmacy team, the role of the Senior Clinical Practice Pharmacist is pivotal to improving the quality of care and ensuring patient safety.
The successful candidate will have a minimum of five years post graduate experience, be an independent prescriber with primary care prescribing experience, and will have completed the Primary Care Pharmacy Education Pathway.
The post could be worked as full time over five days, or part time - minimum of 22.5 hours over three days.
Competitive salary, negotiable depending on experience and skillset. 5.6 weeks pro rata annual leave per year with one week study leave.
Main duties of the job
Leadership of the Practice Pharmacy Team- ensuring the team delivers a comprehensive high-quality service to the patients and staff of the practice
Patient facing roles seeing patients and managing medication related problems and queries. Medication reviews at point of care transfer, especially for older frail, care homes and polypharmacy
Staff facing roles - providing support to general practice staff in relation to repeat prescriptions and medication queries
Quality, governance and improvement - working with the prescribing and wider practice team on safety and medicines optimisation. Leading on initiatives, medicines audit work and local projects and schemes of work
The team manage a complex and varied program of working across all aspects of medicines management. This includes day to day and more developmental and strategic work. The post holder would lead this work along with quality improvement, audit and development of services. We work closely with the ICB Clinical Pharmacist.
Regular clinical supervision by a GP partner with opportunities to grow and develop professionally.

Job responsibilities
The post holder is an experienced clinical pharmacist with previous experience of working in Primary Care, who acting within their professional boundaries and as part of the wider clinical team, will develop and manage medicine management services within the practice.
The post holder will lead the multi-disciplinary pharmacy team, to meet practice service requirements for medicines management.
The postholder will manage systems and be part of the workforce relating to repeat prescribing, authorisations and reauthorisation, acute prescription request, management of medicines on transfer of care and systems for safer prescribing.
The post holder will lead processes and perform medication review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities.
The post holder will provide leadership on medicines optimisation, leading on local ICB projects and schemes of work, quality improvement and clinical audit.

Qualifications Essential
Mandatory registration with the General Pharmaceutical Council (GPhC) Masters degree in pharmacy (MPharm) (or equivalent) Independent prescriber
Desirable
A member of or working towards Faculty membership of the Royal Pharmaceutical Society Specialist knowledge acquired through post- graduate diploma level or equivalent training/experience
Experience Essential
