About this role

Job summary

Invicta Health has an exciting opportunity for a Clinical coder and summariser to join the team on a part time basis,30 hours per week.

We reserve the right to close this vacancy early if we receive sufficient applications for the role.

Main duties of the job

As a Clinical Coder and Summariser, you will support the practice in becoming a paper-light environment by summarising and read coding relevant information from patients clinical records onto the practice computer system in adherence to the Practice Summarising Protocol. You will extract information from patients notes and enter it onto their digital record, displaying and significant medical problems prominently.

Job description Job responsibilities

Summarise and code patient records onto their digital records using read code/SNOWMED codes to a high standard and according to set protocols and timescales agreed by the practice.

Review medical records and extract information to produce an accurate summary of the patients medical history.

Use DOCMAN to enter QOF data, ensuring correct SNOMED clinical codes are used

Checking and integrating medical records received electronically.

Examining records and documentation for missing information, using medical knowledge to correct mistakes found in existing patient records, including degraded codes.

Identify and highlight any read coding problems to the GP and/or Practice Manager.

Adhere to the Summarising Protocol and keep update with any changes.

Safeguarding

Attend clinical meetings where required, communication any safeguarding updates and review of safeguarding registers.

Manage and maintain safeguarding record.

Communication

Communicating with other practices and healthcare professionals regarding patient documents.

Collaborating with clinical colleagues and other healthcare professionals to ensure information accuracy.

Ensuring that the practice complies with clinical coding guidelines including alerting other users of new guidelines or changes to coding protocols.

Process E-Letters as required on DOCMAN.

Data Handling

Undertake filing and retrieving of paperwork ensuring up-to-date maintenance.

Analyse patient records, extracting and accurately recording all relevant clinical information.

Accurately code information, following coding protocol and using industry recognised codes.

Examine records and documentation for missing information and ensure correct coding.

Collaborate with clinical colleagues and other healthcare professionals to ensure information accuracy.

Use DOCMAN to enter QOF data, ensuring correct SNOMED clinical codes are used.

Carry out administrative tasks relating to patient records, including filing, and document management.

To contribute to the development of services, including processes, templates and principles.

To take part in any statutory and mandatory training or other training and development programmes as requested.

Person Specification

Experience Essential

Understand the importance of maintaining patient and data confidentiality. Able to use judgement and experience in helping to determine the relevant urgency of requests for information received from patients and other external organisations. Experience of setting up and maintaining effective administrative filling systems Experience of work involving regular and accurate data inputting

Desirable

Knowledge of office management systems and procedures Experience of working with Microsoft packages including, word, excel and power point Experience of working with EMIS and DOCMAN Previous administration experience within a medical environment Knowledge and understanding of the National Health Service

Qualifications Essential

Good level of education including competent levels of numeracy and literacy
