Job summary
We are looking for a registered Pharmacy Technician with current clinical knowledge, a patient centred outlook and experience in a primary care setting to join our large Pharmacy. Team. The successful candidate will work alongside a senior clinical pharmacist and will have protected time for supervision and education.

We are looking to welcome an experienced Clinical Pharmacist into our PCN who will play a full and active role as part of the practices multidisciplinary team.

Job description Job responsibilities
Prescription Queries and Medication Reconciliation
Work closely with the pharmacy team to ensure coordinated and efficient processing of prescription queries.
Support the processing of repeat prescriptions including Nursing & Residential Home monthly prescriptions and requests together with the admin and pharmacy team.
Work alongside clinicians to carry out medicine reconciliations for discharge summaries, clinic letters and other transfers of care information and communication in line with Practice policy.
Liaise with primary and secondary care colleagues and others to resolve prescribing queries
Update and maintain accurate and comprehensive patient medication records
Improving Patient Safety Audits and Supporting Reviews
Work alongside the pharmacy team to monitor and action safety and drug alerts in a timely and appropriate manner.
Work with clinicians to develop and undertake medicine review audits and other quality improvement project work as required
Work with clinicians to undertake regular analysis of prescribing data and provide Practice updates under guidance of Practice Pharmacist
Work alongside the admin team to promote patients to attend appointments for long term conditions
Working in a wider team
Develop positive and professional working relationships with colleagues in local pharmacies and in hospitals to ensure good patient care
Undertake any other additional duties appropriate to the post as requested by the Partners or the Clinical Services manager Practice Pharmacist.
To respond to medication queries from patients and staff in a professional manner referring to the appropriate clinician in accordance with practice protocols
Work to support the achievement of prescribing outcomes for Quality Outcomes Framework.
Undertake any other additional duties appropriate to the post as requested by the Partners or Lead Clinical Pharmacist.
Person Specification
Qualifications Essential
BTEC Level 3 Diploma in Pharmaceutical science or NVQ Level 3 Diploma in pharmacy service skills or equivalent with underpinning knowledge Current registration as a pharmacy technician with the General Pharmaceutical Council
Experience Essential
A minimum of 2 years post qualification experience Up to date clinical pharmacy therapeutic knowledge and practical knowledge of medicines optimisation principles and evidence based medicine Knowledge of pharmacy law and ethics, medicines optimisation, including legislation and policy Awareness of systems to support management of patients in a primary care setting, delivering pharmaceutical input and support in the context of pathways of care and the business of the practice Understanding of the NHS agenda and the issues affecting primary care Experience of change management and putting ideas into practice Experience dealing with difficult situations Experience of completing clinical audits
Desirable
Experience as a primary care pharmacy technician or part of a primary care medicines optimisation team An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing Experience of analysing prescribing data Knowledge of EMIS clinical system
