Job summary

Chiswick Family Practice is seeking a dedicated, motivated, enthusiastic and forward-thinking General Practitioner to join our friendly practice team. This is an excellent opportunity for a motivated GP who is passionate about delivering high-quality patient care within a supportive and collaborative primary care environment.

The postholder will primarily undertake patient-facing clinical work, contributing to the delivery of safe, effective, and responsive general practice services to our registered population. The role also offers opportunities to contribute to wider practice initiatives and quality improvement activities.

Main duties of the job

Assess and triage patients, including same-day presentations, providing treatment or referrals in line with NICE guidelines and local pathways. Manage undifferentiated and complex conditions through history taking, examination, diagnosis, and care planning. Deliver face-to-face and telephone consultations, supporting long-term conditions, acute presentations, and undertaking clinical administration (prescriptions, referrals, and test results). Provide high-quality, evidence-based care and prescribe in line with national and practice policies. Support vulnerable groups, including care home residents, and identify and manage mental health needs with appropriate referral. Accurately interpret pathology results and ensure effective use of read codes. Assist with clinical emergencies, safeguarding, and medicines optimisation. Contribute to QOF and practice targets, liaise with external services, support personalised care plans, and undertake home visits where required.

Job description Job responsibilities

In accordance with the practice timetable, the GP will work autonomously, delivering care within their scope of practice to the patient population. Contributing towards clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients Maintain accurate, contemporaneous, and complete patient records Work within professional competence and GMC guidelines Deliver care in accordance with CQC standards and best practice guidance Prioritise and manage workload effectively to maintain high standards of care Work collaboratively within the multidisciplinary team Provide supervision and support to other healthcare professionals where appropriate Contribute to practice meetings, clinical discussions, and service development Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety. A commitment to life-long learning and audit to ensure evidence-based best practice. Participate in local initiatives to enhance service deliver and patient care. Participate in the review of significant and near-miss events Attending training and events organised by the practice or other agencies, where appropriate.

The post-holder will adopt a full range of promotion and management of their own and others health and safety and infection control as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include (but will not be limited to):.

Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines. Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others. Hand hygiene standards for self. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process. Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes. Safe management of sharps use, storage and disposal. Maintenance of own clean working environment. Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management. Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised. Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with responsible managers. Undertaking periodic infection control training. Correct waste and instrument management including handling, segregation, and container use. Maintenance of sterile environments. Qualifications Essential
