Lower Broughton Health Centre & The Energise Healthy Living Centre is a progressive and patient-focused GP practice committed to delivering high-quality healthcare to our local community. We are seeking an enthusiastic, organised, and detail-oriented Clinical Coder to join our friendly and supportive administration team.
This is an excellent opportunity for someone with experience in clinical coding within a primary care setting who is looking to further develop their skills in a modern and forward-thinking practice.
Main duties of the job
The successful candidate will play a key role in ensuring the accuracy and quality of patient records by reviewing and processing incoming clinical correspondence. You will be responsible for data entry of medical information and assigning appropriate SNOMED codes, helping to maintain high standards of data quality and supporting the practice in achieving clinical and contractual targets.
Key Responsibilities
Review, analyse and process incoming clinical correspondence, both electronic and paper based. Accurately assign SNOMED codes to diagnoses, procedures, investigations, test results and hospital correspondence. Process and workflow documents within EMIS Web to the appropriate clinicians and healthcare professionals. Maintain high standards of data quality and record accuracy to support patient care, audits, QOF achievement and NHS contractual requirements. Identify and resolve coding discrepancies in collaboration with clinicians and administrative colleagues. Support the ongoing improvement of clinical coding processes and data quality initiatives within the practice. Adhere to information governance, GDPR and confidentiality requirements at all times.

Job description Job responsibilities
Clinical Coding & Data Entry
Accurately code clinical information from hospital letters, clinic correspondence, and pathology results using SNOMED CT (or Read codes where applicable). Ensure diagnoses, procedures, vaccinations, allergies, and medications are correctly coded. Process incoming electronic and paper correspondence in a timely manner. Maintain accurate problem lists and patient summaries. Data Quality & Record Maintenance
Identify and correct coding inconsistencies or missing data in patient records. Ensure long-term condition registers (e.g., diabetes, hypertension, asthma, COPD) are accurate and up to date. Support data cleansing activities and periodic audits of clinical records. Work with clinicians to clarify ambiguous clinical information. Quality & Performance Support
Support achievement of QOF, enhanced services, and local incentive schemes through accurate coding. Assist with recall systems and disease surveillance lists. Ensure accurate coding for immunisations, screening, and health checks. Systems & Administration
Use the GP clinical system EMIS Web Follow practice protocols for document management and coding workflows. Maintain confidentiality and comply with GDPR and data protection requirements. Communication & Teamwork
Liaise with GPs, Managers, nurses, pharmacists, HCAs, and administrative staff regarding coding queries. Provide feedback on recurring coding issues or system improvements. Support training of staff on basic coding principles where required.
Person Specification
Experience Essential
Previous experience in clinical coding and medical administration within a UK General Practice or NHS setting. Good understanding of medical terminology and clinical correspondence. Experience using EMIS Web or similar primary care clinical systems. Excellent attention to detail and a high level of accuracy. Strong organisational and time management skills. Understanding of patient confidentiality, GDPR and safeguarding responsibilities. Ability to work independently and as part of a multidisciplinary Team
Desirable
Formal clinical coding qualification or relevant training. Experience supporting QOF, enhanced services and data quality projects. Knowledge of NHS primary care contracts and reporting requirements
