Job summary
We are seeking enthusiastic Clinical Independent Prescribing Pharmacist to join our established team and be a part of a company that really makes a difference.
As a GP Practice Pharmacist, you will work within a GP practice providing patient-facing dispensing, advice, treatment and administrative support.
Our Clinical pharmacists work as part of the general practice team and are supported by, and work with, a diverse multi-skilled professional team of clinical and non-clinical friendly staff, providing high quality, person-centred, holistic care to patients registered with Omnes Healthcare practices.
Main duties of the job
Conducting patient consultations and clinical assessments. Prescribing and reviewing medicines. Managing long-term conditions such as diabetes, asthma, COPD, and hypertension. Carrying out structured medication reviews. Monitoring high-risk medicines and ensuring safe prescribing. Responding to acute medication requests. Reconciling medicines following hospital discharge. Providing medicines advice to patients and clinicians. Auditing prescribing practices and supporting medicines optimisation.

Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
