Job summary
Flixton Road Medical Centre in Trafford is providing an exciting opportunity for a Clinical Pharmacist to join a proactive GP Practice covering over 12,000 patients.
The ideal candidate will be a forward-thinking, enthusiastic individual with initiative who is keen to work within a team of like-minded people and deliver excellent pharmacy-led services to our population in West Trafford.
You will become part of an experienced team with clinical support from the Lead GP.
Working within the practice, you will work closely with GPs, nurse, assistant practitioners, social prescribers and medicines management as part of a multi-disciplinary team while also building strong effective working relationships with the local community, hospitals and wider pharmacy teams to improve the care and services to our patients.
The role will focus on both practice and PCN initiatives as set out in the GMS contract, QoF, PCN contract, NHS Long Term Plan and deliverables agreed by the GP partners to improve place based patient care.
Main duties of the job
Management of patients with chronic diseases to proactively manage patients with complex polypharmacy
Clinically assess and treat patients using their expert knowledge of medicines for specific disease areas
Provision of specialist expertise in the use of medicines whilst helping to address public health and social care needs and tackling inequalities across Trafford
Development of relationships and close working with pharmacy professionals across the wider health and social care economy
Provision of leadership on person-centred medicines optimisation and quality improvement
Delivery of first-class, professional service to support patients and GP practice
Proactive delivery of QoF and other locally enhanced targets.

To support Flixton Road Medical Centre in all aspects of medicines optimisation.
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation within the practice. This will include structured medication reviews with direct patient contact and may include contributing to
Management of long-term conditions
Minor illness
Management of medicines on transfer of care
Reviewing systems for safer prescribing
Contributing to repeat prescription authorisations and reauthorisation.
Action acute prescription requests
Addressing both the public health and social care needs of patients
Contributing to achievement of QOF and locally commissioned quality improvement schemes.
Undertaking clinical audit
Developing, promoting and implementing the high quality, evidence based and cost effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial).

Person Specification
Experience Essential
Commensurable experience working at foundation level of the RPS pharmacy framework, demonstrated within practice portfolio. Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice. Demonstrates ability to integrate general practice with community and hospital pharmacy teams and community groups. Post registration experience minimum of two years. Experience of working in the NHS. Experience of communicating and engaging effectively with internal and external agencies/ stakeholders. Experience of planning complex medicines management projects and programmes.
Qualifications Essential
Professional knowledge acquired through vocational masters degree in pharmacy (4 years) or equivalent + 1 year pre-registration training. Holds or working towards an independent prescribing qualification Successful completion of/working towards completion of the CPPE Clinical Pharmacists in General Practice Education Programme. Demonstrates an understanding of, and conforms to, relevant standards of practice. Follows professional and organisational policies/procedures relating to performance management
