Job summary

The post holder will work across Holbrook Surgery and Riverside Medical Practice with each of the practice teams, joining the PCN team for Central Horsham. They will be assigned a lead clinical and operational contact for each contract. ABC as employer will provide support and ensure access to the NHSE England training programme as required.

This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England.

The paramedics will take referrals direct from practices to carry out in practice clinics, home visits, telephone or online triage as appropriate. The role will involve the assessment, diagnosis, treatment or referral of patients presenting with undifferentiated and undiagnosed conditions. The post holder will work within their level of competency and will be clinically supervised by a GP working in the area.

This role will have completed or be working toward a Band 7 FCP Paramedic role, based on the completion of stage 1 and stage 2 of the Health Education England Competency Framework. You will then be assisted to develop and enter the ACP pathway. We will offer a fully funded ACP training package with a dedicated GP mentor and an Advanced Clinical Practitioner contract at the end of the MSc ACP. You will be supported through indirect supervision and will be responsible for assessing, diagnosing, interpreting investigations and treating patients within Primary Care within a developing scope of competence.

Main duties of the job

Horsham Central PCN are looking for a Paramedic to join their innovative and dynamic Primary Care Network to primarily focus on supporting them with their community and housebound patients. You will be provided with a range of clinical supervision and training opportunities to develop your skills in Primary Care.

You will work closely with clinicians from the practices and will play a vital role in increasing capacity for all appointments including home visits. The successful candidate will be provided with excellent support, the opportunity for development in the role, and will be clinically supervised by a General Practitioner.

As a Paramedic, you will be required to work autonomously in the community across the network practices, providing a range of services such as assessment, diagnosis, treatment, telephone triage, clinical decision-making, and referring patients appropriately under supervision.

You will be predominantly community based, undertaking home visits, and liaising with a multidisciplinary team including GPs as well as other services. The post holder will start their day at one of the surgeries and may be required to rotate accordingly.

The post holder will work within their level of competency and will report to a GP/Partner who will provide support and mentorship.

To provide assessment, diagnosis and treatment at first point of contact by attending to patients according to patients needs either in the Practice or patients homes Work with practice teams to ensure the process for referral of urgent on the day patients is smooth, and appropriate patients are referred making full use of the practitioners skills To accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history To evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis. To use broad clinical guidelines (NICE etc) relating them to individual cases, providing assistance and feedback to the organisation and others on how these should be interpreted To signpost appropriately to other services in the identified healthcare pathway To refer patients to appropriate care settings, within locally agreed healthcare pathways To advise and support members of the public on healthy lifestyles, health promotion and disease prevention To maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse eventsCommunication

Knowledge, Skills & Experience Essential
