About this role

Job summary

We have a substantive Band 6 Community Mental Health Practitioner post within Suffolk Community Rehabilitation Team open to applications from Occupational Therapists, Registered Mental Health Nurses and Social Workers . The team is based in Ipswich, working with people open to the local community mental health teams with a diagnosis of complex psychosis. We are an Occupational Therapy led service, offering rehabilitation interventions to Service Users and their carers in the community, and in-reaching into our acute and rehabilitation wards. The team is a specialist multi-disciplinary team providing time limited assessment and interventions as well as carers group and Family Intervention. The team has OTs, mental health nurses, a peer support worker and a psychologist and works closely with the inpatient rehab ward (SRRS).

You will be a qualified professional with experience of supporting people with psychosis and their families, preferably in the community. You will demonstrate a strong understanding of the impact of complex psychosis on peoples lives and the value of therapeutic intervention to enhance quality of life.

Main duties of the job

The team currently operates as an intervention service, with care coordination remaining with the CMHT. You will have cases allocated to you as a rehab keyworker.

The typical job roles and responsibilities will include:

Working with other multi-disciplinary professionals within the team. Undertake initial assessments. Liaise with statutory and voluntary sectors The successful candidate will be an integral part of this team. You will contribute to meetings both within the service and in wider networks. Effective personal planning and organisation of daily workload. Flexible working and manage own working time effectively and accordingly to service demands. Managing and providing a high quality of care. Managing a case load of patients that have complex psychosis. Initiate, create and maintain therapeutic relationships with patients and carers, ensuring that practice is patient centred. Practicing in line with policy, guidance and legislation. Carrying out risk assessment Working closely with the ACS mental health team to support our patients, meeting both their mental health needs and their Care Act needs.

Job description Job responsibilities

The post will involve you working in the community. You will require effective communication and ability to be creative in planning how to engage with your caseload. You will be undertaking initial rehab assessments, liaising closely with CMHT staff, supported accommodation providers and families. As a band 6 you will be expected to contribute to service development and supervise others.

Qualifications Essential

Registered Mental Health or Learning Disability Nurse, Occupational Therapist OR Social Worker with the relevant professional registration Ongoing professional development working towards degree level ( if professional registration is following a diploma level course)

Desirable

Higher degree or post graduate certificate in area of clinical / therapeutic practice

Experience Essential

Experience in the delivery of clinical interventions Experience in working in team delivering person centred treatment packages

Desirable

Experience in delivering group based interventions

Skills Essential

Competent assessment skills Competent and effective documentation and record keeping

Desirable

Prevention and Management of Suicide training Safeguarding training

Knowledge Essential

Up to date knowledge of legal and ethical frameworks including Mental Health Act

Ability and willingness to travel independently to meet the needs and requirements of the service
