Job summary

LADMS is seeking a highly motivated and experienced Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join our dynamic and patient-focused team. Please note: This role is for a bank position within LADMS Staffing which supports multiple clinics and contracts across all days. Some clinics are remote working opportunities. Shifts are published online and are equally shared amongst staff who apply for them.

Administration support is provided by centralised LADMS Headquarters team.

Main duties of the job

ANP/ACP will work autonomously in primary care settings, community clinics and private healthcare settings. ANP/ACP will also be able to work remotely and independently.

ANP/ACP will be able to work in busy and thriving healthcare environments across a multitude of services and sites.

ANP/ACP will be able to prescribe appropriately and adhere to all guidelines.

LADMS Staffing is a branch of LADMS (Lincolnshire and District Medical Services) which currently holds several NHS Contracts across Enhanced Access and community clinics, supports one GP Surgery and also offers private healthcare as part of Private GP Lincs and in conjunction with third party private healthcare providers.

Job description Job responsibilities

The Advanced Clinical Practitioner (ACP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The post holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills and behaviours appropriate to the healthcare professionals (HCP) setting and job role.

An ACP is autonomous in making decisions based on assessment, diagnosis and interpretation of test results and will initiate, evaluate, and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

To provide timely and appropriate care for patients with suspected acute respiratory infections and reduce pressure on other parts of the Lincolnshire healthcare system by providing same-day access for adults and children who require face to face assessment. Treatments will include the prescribing of antivirals, antibiotics, etc.

2. Main duties/primary responsibility:

The following are the core responsibilities of the Advanced Clinical Practitioner.

There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice

b. Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information

c. Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change

d. Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate

e. Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change

f. Use expertise and decision-making skills to inform clinical reasoning

g. Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care

h. Exercise professional judgement to manage risk

i. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

j. Implement and evaluate individual specialised treatment plans for chronic disease patients

k. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

l. Be a professional role model for the organisation

m. Actively engage in peer review to inform own and others practice, formulating and implementing strategies to act on learning and make improvements

n. Actively seek feedback and involvement from individuals, families, carers, communities and colleagues in the co-production of service improvements

o. Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice (clinical practice, leadership and management, education and research).

p. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

q. Maintain accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively

r. Develop, implement and embed an effective call/recall system

s. Critically appraise and synthesise the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others

t. Develop and implement robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance

u. Chaperone patients where necessary

v. Support the team in dealing with clinical emergencies

w. Support the team with all safeguarding matters in accordance with local and national policies

x. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

y. Deliver opportunistic health promotion where appropriate

3. Secondary responsibility

In addition to the primary responsibilities, the Advanced Clinical Practitioner may be requested to:

a. Support the IPC lead, ensuring compliance, and undertaking audit as necessary

b. Monitor and ensure the safe storage, rotation and disposal of medicines

c. Undertake overall management of a clinical team, providing guidance when necessary, acting as a mentor to students and newly qualified staff

d. Maintain an effective clinical staff rota ensuring all clinics are staffed appropriately

e. Participate in local initiatives to enhance service delivery and patient care

f. Support and participate in shared learning within the organisation

g. Develop an area of specialist interest, taking the lead within the organisation

h. Continually review clinical practices, responding to national policies and initiatives where appropriate

i. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)

j. Commensurate with their role, support the development of clinical services within the organisation, liaising with external agencies and professional organisations as required

k. Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

Person Specification

Qualifications Essential

Qualified healthcare professional Educated to Masters degree as detailed within the HEE document tilted Multi-professional framework for advanced clinical practice in England Qualified independent prescriber - V300 Registered on their appropriate professional register

Teaching qualification ALS and PALS

Experience Essential

Experience of practice within the four pillars Job plan that demonstrates advanced clinical practice and has equity with peers working at this level

Experience of prescribing and undertaking medication reviews Experience of working in a primary care environment

Clinical knowledge and skills & Other Requirements Essential

Clinical knowledge and skills [To have a good understanding of national guidelines for ILD, COPD and asthma, Oxygen including diagnosis and evidence based interventions To assist in the care respiratory patients including assessment, care planning, and on-going support, monitor adherence, including inhaler technique, untoward side effects of the drugs.
