About this role

Job summary

We are delighted to invite applications for a number of Clinical Fellow posts within our expanding Frailty Hospital @ Home (H@H) Virtual Ward service. This is an exciting opportunity to join a forward thinking, community based team delivering high quality, patient centred acute care outside the traditional hospital setting.

Contract Type: Fixed Term: 5 August 2026 -- 3 August 2027

Hours: Full Time - 40 hours per week

Working Pattern: 08:30--16:30, Monday--Friday + weekend rota covering the working hours 0800-2000

On Call: There is no on call requirement for this position

Base: Community / Trust-wide

Main duties of the job

As a Clinical Fellow, you will play a key role in the clinical management of frail and medically unwell patients within their own homes. Working as part of a dedicated Virtual Ward team, you will provide safe, effective care supported by:

Senior medical supervision

Advanced Clinical Practitioners

Specialist Nursing staff

A wider multidisciplinary team

The post offers a high degree of autonomy, and you must be confident working with indirect supervision and undertaking lone working in the community.

Weekend participation in the H@H rota is required.

Given the significant focus on heart failure management, we also welcome applicants with an interest in Cardiology.

Applicants should ideally have previous NHS experience, either through employment or a clinical attachment.

This is a unique opportunity to gain hands on experience in an innovative model of acute care, develop strong clinical decision making skills, and work closely with a supportive multidisciplinary team. You will gain valuable exposure to frailty, complex comorbidity, and heart failure management -- all highly relevant to future training applications.

Person Specification

Qualifications Essential

Medical Degree: BSc, BA with grade 1 or 2:1, or equivalent Full GMC with a Licence to Practice

Desirable

Medical Degree: BSc, BA with grade 1 or 2:1, or equivalent Full registration and license to practice with GMC

Experience Essential

Completion of foundation training or equivalent, including evidence all clinical skills. Experience of working in an NHS hospital, other than that of Clinical Attachments

Desirable

Teaching of junior medical staff, nursing staff Demonstrate a commitment to the specialty

Additional Criteria Essential

Evidence of participation in audit / quality improvement locally and understanding of the principles Appropriate skills for level of experience Ability to work well in a team and under pressure Good time management Experience in the assessment and management of unselected medical emergencies at foundation level Experience of acute medical speciality at foundation level Evidence of a good standard of Literacy / English language skills Ability to communicate well with patients, their relatives and colleagues Willing and able to work with colleagues in management, nursing and other specialities to ensure the delivery of a high-quality, safe service Familiar with clinical governance Good IT skills
