About The Role
We're looking for a passionate and proactive Health and Wellbeing Practitioner to join our team at HMP Cardiff on a full-time basis (37.5 hours per week). This is an opportunity to support individuals on their journey towards positive change, helping them improve their health, wellbeing, and prospects for a successful return to the community.
You'll manage a varied caseload, deliver tailored interventions, and work collaboratively with service users to develop meaningful care and release plans that support long-term recovery and wellbeing.
What You'll Be Doing
Managing a caseload of service users and coordinating their treatment journeysConducting assessments and developing person-centred care plansDelivering structured 1:1 interventions and facilitating group sessionsProviding harm reduction advice and wellbeing supportSupporting release planning and through-care arrangements for individuals returning to the communityCarrying out second signatory duties and supporting clinical servicesWorking closely with multidisciplinary teams and partner agenciesMaintaining accurate case records and performance data to meet service targets What We're Looking For
Experience supporting individuals with complex health, wellbeing, substance use, or related needsStrong assessment, case management, and communication skillsAbility to build positive relationships and motivate changeExcellent organisational skills and attention to detailConfidence working in a fast-paced, target-driven environmentA collaborative and solution-focused approach This is more than a job, it's a chance to play a vital role in rehabilitation, recovery, and reducing reoffending. All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
