About this role
This is an exciting opportunity to join the Strategic Commissioning Team at Cornwall Council with priorities for the implementation of new models of Intermediate Care and Carers Support Services. These services will support people to live in their place of choice for longer, support timely hospital discharge and ensure there is support for informal Carers across Cornwall.
