About this role
The Senior Community Health Worker is responsible for helping community residents and Providence Medical Foundation patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The Sr. Community Health Worker will support in the following areas: a) Health Education: Assist with recruitment and administrative tasks for health education courses, such as but not limited to Mental Health First Aid, Creating Healthier Attitudes Today, physical activity, or Groceryships. In addition they will deliver these modules these health education topics independently and identify new locations where the programs can be delivered.
