The LPN is part of the clinical team maintaining t h e f l o w o f t h e p r o v i de r sch e du l es d u r i n g p a t i e n t c a r e s e s s i on s , m i n i m i z i n g p a t i e n t w a i t t im e w hene v e r p o ss i b l e . The LPN p r o v i de s assistance t o providers f o r p a t i e n t c a r e f o ll o w u p n eed s . The LPN may participate in phone calls and/or in person visits with patients to document symptoms, questions, or concerns. This information will be relayed to the RN or provider for the determination of any action needed. The t r i a g e o f c a r e s i t u a t i o n s is the responsibility of the RN or provider ; sc h edu li n g o f a c u t e c a r e appo i n t m e n t s can be facilitated by the LPN. The primary job duties of this position are: Salary Range: $25.79-$33.20/hourly The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. .
