The Medical Social Worker provides psychosocial assessments, crisis intervention services and counseling to patients. This position coordinates healthcare services throughout the continuum of care on behalf of the patient and assists in discharge planning. This position requires population specific competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. The Medical Social Worker responsibilities include, but are not limited to, the following:
• Conduct psychosocial assessments (emotional, cultural, financial needs) • Assist with evaluation and referral for suspected abuse/neglect • Implement social service interventions; counsel patients/families on illness, treatment, and grief/conflict • Assist with advance directives and durable power of attorney for health care • Collaborate with Case Managers and care teams on discharge planning • Act as liaison between patients, families, care teams, and external agencies; facilitate care planning • Maintain required documentation in accordance with policies • Report to assigned manager or director • May require additional physical demands or respiratory fit testing • Follow standard precautions due to potential exposure to infections, blood/body fluids, and chemicals INTEGRIS Health is an Equal Opportunity/Affirmative Action Employer. REQUIRED QUALIFICATIONS EDUCATION:
• Masters in social work (MSW) from an accredited school Hospice and Home Health:
• This job requires the incumbents to operate an INTEGRIS-owned vehicle OR personal vehicle (non-INTEGRIS-owned) and therefore must have a current Oklahoma State Driver's License as well as a driving record which is acceptable to our insurance carrier.
