We are seeking an experienced professional to lead our Next Steps Critical Time Transition Program (CTTP) team. This innovative program is dedicated to supporting youth ages 11–17, who experience discharge delays from hospitals, Emergency Departments and Comprehensive Psychiatric Emergency Programs (CPEPs), in their transition from hospital settings to the community. As the Director, you will guide a multidisciplinary team, ensuring seamless support for youth and their families during these critical transitions.
