Job summary
Experienced clinical pharmacist required based mainly at an intermediate care unit in the north of Birmingham but with some working required at our hospitals at Moseley Hall and West Heath, plus with on call covering the whole trust.

You will be based mainly at Perry Trees intermediate care centre and will be working with a senior pharmacist and pharmacy technician to provide pharmaceutical care to the residents

Job description Job responsibilities
The main duties will include medicines reconciliation, medication review, professional checking, clinical advice, patient counselling, self - administration assessment and coaching, ensuring medication supplies from community pharmacy. You may also be involved in incident reporting and investigation and training for other healthcare professionals. You will be able to attend MDT meetings and Quality improvement meetings. The role will also include similar work at our 2 hospitals as well as on call duties. On call will cover enquiries from the whole trust and may occasionally involve supply from one of the trust pharmacies.

Qualifications/training Essential
Masters Degree in Pharmacy or equivalent. Registered with the General Pharmaceutical Council. Higher degree/diploma in clinical pharmacy. Evidence of continuing professional development.

Significant experience of working in a dispensing environment. Significant experience of working in with multidisciplinary teams in the management of patients. Ability to work as an autonomous practitioner and to make sound clinical judgements and decisions. Experience of involvement and communication with patients and carers. Relevant experience as a pharmacist practitioner. Experience of delivering training.

Experience of working in a hospital pharmacy service.
Skills and knowledge Essential
Sound knowledge of clinical practice and clinical therapeutics. Able to demonstrate a good understanding of therapeutic options. Good analytical and problem solving skills, and ability to analyse drug usage reports. Experience of producing reports using pharmacy software. Demonstrable use of clinical audit to improve practice. Good understanding of risk management and ability to manage risks in pharmacy. Awareness of and commitment to clinical governance practice. Ability to influence junior pharmacy and medical staff, multi-disciplinary teams and managers. Producing patient information regarding medicines and their therapeutic use.
