Job summary
Due to the recent development of our Assessment and Liaison Services, we are in an exciting position to be able to offer a new and challenging opportunity to join us as a forward-thinking, highly motivated, innovative, dedicated and experienced social worker in our Crisis Resolution and Home Treatment (CRHT) Team in North Lincolnshire.
CRHT is a multi-disciplinary team (MDT) that provide 24/7 urgent clinical triage and advice via the crisis telephone line, as well as conducting urgent and emergency mental health assessments for people in crisis. In addition to this, the team also provides intensive home treatment to those individuals admitted onto the caseload as an alternative to hospital admission. The CRHT also act as gatekeepers for all inpatient admissions, crisis house admissions and facilitate early discharge from the inpatient wards.

Main duties of the job
We are looking for a motivated and skilled Social Worker to join our Crisis Resolution and Home Treatment Team (CRHT) in North Lincolnshire. As part of a 24/7 multi-disciplinary service, you will play a key role in providing urgent mental health triage, crisis assessment, and intensive home treatment as an alternative to hospital admission. You will also act as a gatekeeper for inpatient and crisis house admissions, supporting early discharge and ensuring safe, person-centred care.
Working closely with health, social care and community partners, you will use your expertise in the Care Act, Mental Health Act, and Mental Capacity Act to complete high-quality assessments, formulate risk plans, coordinate care packages, and promote recovery-focused, strengths-based practice. The role includes supporting individuals, carers, and families, while contributing to safeguarding, multi-agency working, and the ongoing development of social work practice within the service.

Appropriate demonstrable experience working with people with lived experience people with mental ill health Experience of undertaking statutory duties under the Care Act & Mental Capacity Act. Demonstrable experience of undertaking assessment of needs for individuals who have complex health and social care needs Demonstrable experience of assessing high levels of risk and formulation of robust risk management plans including the ability to make decisions to ensure safety of individuals and carers. Experience of working within a multidisciplinary setting. Experience in safeguarding (both adult and children) and specialist risk assessment.

Knowledge Essential

Good understanding of Anti-oppressive and Anti- discriminatory practice / Anti-racist practice / Health Inequalities Knowledge of a range of mental disorders, including co-morbid drug, alcohol and personality disorders Good understanding of person-centred care and the NHS Transformation Plan related to mental health services.
Skills Essential
Ability to work independently and with autonomy when required, respond appropriately to risk and ability to escalate as necessary. Ability to proactively formulate Risk Management Plans working with a variety of partners for this purpose. Highly motivated, conscientious with the ability to respond to changing situations to respond to the needs of people with lived experience and carers. Excellent organisation and time management skills to enable a flexible response to conflicting demands resulting in the daily reprioritisation of workload to accommodate service user need. Demonstrates good communication skills. Clear/concise command of English written/oral. IT literature with experience of Microsoft, alongside in-house systems or willingness to work towards this. Specialised skills e.g. appraising, interviewing, counselling, recovery focussed skills, systemic practice.

Willingness to participate in associated research and development. Demonstrate an ability to assist in the educational development and skills of all junior staff
