Job summary
The Practice Nurse position is suitable for a qualified nurse, ideally with post-registration experience in Primary Care, including for example, GP nurses, Community Nurses, Urgent Treatment Centre Nurses, etc.You would be willing to support a wider clinical team on any one of a number of particular clinical areas which have high demand amongst our patient demographic, e.g. Diabetes, Anticoagulation, Tissue viability, Respiratory etc.
The role requires resilience and the ability to be flexible and adaptable. You will be confident to work alone without direct supervision, undertaking and reporting autonomous decisions made in practice.
Main duties of the job
Demonstrate competence and clinical skills in the assessment of patients in the general practice setting, ensuring care is planned in partnership with patients and all information is clearly and objectively recorded according to practice policy.
Able to recognise personal development needs and also facilitate learning for patient, carers, other staff and students.
You are part of the Mereside practice nursing team with delegated responsibility for patient care, being accountable to the Nurse Manager.

To do this you will be required to:
Practice in compliance with your respective code of professional conduct and scope of practice, being responsible and accountable for your decisions, actions, and omissions at this level of practice Be competent in conducting the full range of responsibilities typically associated with Treatment Room and Primary Care, including: Child Immunisations,Cervical Cytology,Complex Dressings Assess patients, taking into account their physical, mental and social states, alongside the impact of their environment and support available to them, to produce a person-centred plan of care Demonstrate knowledge of a broad range of long-term conditions and local care pathways. Demonstrate effective communication skills to optimise patient satisfaction while managing their expectations and supporting them to make decisions, plan care, or make positive change, including: When dealing with sensitive or confidential matters that may also have legal or ethical considerations, for example, safeguarding, substance abuse, sexual orientation and gender identity, racial and ethnic diversity, mental illness, disabilities, etc. To support patients to adopt health promotion strategies, and apply principles of self-care, including referring or sign-posting to other services that may support their wider well-being, e.g. District Nurses, Social Prescribers, Health & Well-being coaches, etc. Work collaboratively with your colleagues within Mereside practices and across the local health economy to manage risks and issues, and integrate effectively with health and social care providers, ensuring consistency of care across settings and practices Actively contribute to implementing the quality governance systems and processes across the organisation, demonstrating receptiveness to challenge and learn, and preparedness to constructively challenge others, in doing so. This includes but is not limited to healthcare records, learning points, and significant events: Be confident in, and/or willing to learn, relevant technologies to facilitate remote consultations and effective cross-site working (e.g. MS Teams, AskMyGP, SystmOne, AccuRx) Be willing to develop learning materials for patients and adapting care to support individual needs in patients, across a variety of media.Facilitation and Learning
