Join us as a Band 3 Community Healthcare Support Worker, Full time or part time, in a substantive position. Our service operates from 5pm to 8am Monday- Sunday

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Main duties of the job

We are looking for Community Healthcare Support Workers to join our team who are committed and passionate about promoting health, well-being and independence. We are looking for a dedicated and enthusiastic clinician who believes in the provision of patient-centred care.

A key aim of our service is to optimise patient care within their own homes and reduce unplanned admissions.

Collaboration is key in Nottinghamshire and we work closely with colleagues across the region to provide a holistic, multidisciplinary approach to care delivery.

Job description Job responsibilities

You should possess good interpersonal and time management skills, be a good communicator, organised and able to prioritise and work well as a member of a team. Supporting community nurses when required. in patients homes. You must be able to demonstrate a professional manner at all times to patients, relatives and staff of all levels.

Part time hours are available between 24 and 36 hours per week. Evening shifts are 5pm-11pm and night shifts are 7pm - 8am. You will be required to work a combination of both shift patterns and cross over into the South Nottinghamshire areas when required.
