An exciting opportunity has arisen to join the NHS Wales award winning Stay Well @ Home Team at the prince charles hospital.
We are looking for a skilled, organised and enthusiastic individual to join our established and friendly, integrated team of Occupational Therapists, Physiotherapists, Social Workers and Assistant Therapy Practitioners (ATP's).
Main duties of the job
Key aspects of the role include working as part of a multi-disciplinary team to undertake specific clinical/therapy based interventions, administrative tasks and liaison with local authority and third sector based community services to promote and support timely discharges from the Accident and Emergency (A&E) and the Acute Medical Unit as appropriately delegated by the registered clinicians.
As part of the team, the ATP plays a vital role in providing high quality care and interventions for our patients with the service aimed at reducing length of stay, improving admission avoidance and reducing avoidable readmissions through partnership working with community based services. Whilst this post is predominantly hospital based, there will be opportunity to undertake community based visits where required. You will be able to support the assessment and discharge planning of people with a range of functional difficulties within the bounds of the ATP role. Patient management will include responsibility for the delivery of care including liaison with patients, family, members of the MDT and external agencies. You will also need to have excellent time management and prioritisation skills and the ability to communicate effectively both verbally and in written form.

Knowledge Essential
Knowledge relating to a range of core OT/PT activities Knowledge of the discharge planning processes, specific conditions and forms of therapy
Qualifications Essential
Evidence of good general education, English/Mathematics GCSE or equivalent qualification NVQ level 3 OCN/Agored accredited modules or relevant NVQ level 3
Desirable
Expected to achieve competency based training within a two year period.
