This post offers an exciting opportunity for a suitably qualified and experienced Nurse to work within the Specialist Coronary Heart Disease & Heart Failure Team based at Innovation House in Bishop Auckland covering the Dales area. The post covers core hours of 09:00-17:00 Monday-Friday with currently no weekend working although that may change in the future.
Main duties of the job
To case manage patients with a cardiac diagnosis in hospital or at home without direct supervision, reporting to the cardiovascular specialist nurses or wider multi-disciplinary team as appropriate.
To contribute to the development of service specific guidelines and protocols that are evidence based.
Assess patient need and develop a plan of care that is individualised and meets the needs of patients and carers.
Work closely with key members of the team from primary and secondary care to alter the management plan as appropriate. Communicate any document any changes in a timely manner.
To deliver cardiac and heart failure rehabilitation across the boundaries of County Durham and Darlington in line with agreed protocols.
To support service provision within the local area and wider foundation trust.
Assist patients to identify and manage risk factors using behavioural change to self-manage their condition.
Monitor and manage adjustments in medication within agreed service protocols.
To work within a team to deliver specialist care to patients with coronary heart disease and heart failure.
Accepts the responsibility for decisions made and actions taken.

Offering specialist nursing care, support and monitoring to patients with cardiovascular disease and heart failure
Delivering cardiac rehabilitation both face to face in classes and home based (heart manuals and/or digital).
It involves support to optimise treatments prescribed following diagnosis and monitoring effects.
It involves education and support around lifestyle issues and risk reduction
Provides day-to-day supervision and/or clinical supervision to others.
Undertakes specialist assessment, planning, implementation and evaluation of clinical care for caseload of patients/clients with cardiovascular disease and heart failure.
Provides specialist advice to patients/clients/carers
Is an accountable practitioner.
Maintains records as autonomous practitioner.
Skilled at assessing and managing risk.
The job entails independent travel around the Easington/Seaham area with possible need to support other teams across the CDDFT footprint.
Person Specification
Qualifications Essential
Degree level qualification or evidence of study at degree level Active first level Registration with the NMC. Specialist professional knowledge acquired through degree or equivalent level of demonstrable professional knowledge/experience. Supplemented by diploma level specialist training, experience or short course. Sound knowledge of national and regional policies influencing Coronary Heart Disease and Heart failure

Statutory Registration Essential
Nursing Midwifery Council part 1 and 2 Registration Registered Nurse (adult or Child) on part 1 of the NMC register Valid UK NMC PIN
