Job summary

Job Summary
The Care Coordinator will provide a combination of care coordination and practical support for patients across 11 nursing, residential, and learning disability (LD) homes, as well as a dedicated respite care facility. Working closely with the multidisciplinary team, you will help ensure the delivery of high-quality, person-centred care and support the smooth day-to-day running of services.
The role involves regular travel between care homes to support patient reviews, care planning, proactive care management, and communication between healthcare professionals, care home staff, patients, and families.
