Would you like to make a difference to the community you live in and care about? We have an exciting opportunity to join our Health, Equity & Inclusion Team as a Community Support Worker.
Our team hub is based at Charlotte Keel Medical Centre in Bristol, where we meet regularly in person. Wework in partnership with local communities and system partners to address health inequalities and improve wellbeing outcomes.
In this role, you will support our prevention and community engagement approach by working closely with community groups, VCSE organisations and wider partners. You will help build strong relationships, understand local needs and contribute to inclusive and effective service delivery.
We are looking for individuals who are confident at working independently in community settings, with strong communication skills and the ability to build trust and rapport. We are recruiting for two posts: one covering Bristol and one covering North Somerset. We offer full-time, part-time and flexible working options, including availability on either a Tuesday or Thursday.
Main duties of the job
Engage with local communities to build relationships and gain insight into community strengths, needs and priorities. Work collaboratively with VCSE organisations, community groups, and system partners to support joined-up approaches to health and wellbeing. Support the identification and reduction of health inequalities through targeted engagement and inclusive practice. Gather, interpret and feedback community insight to inform service improvement and design. Contribute to preventative approaches that improve long-term health and wellbeing outcomes. Support the development of neighbourhood-based approaches to care and wellbeing. Promote inclusion, equity and accessibility across services and community engagement activities. Support Sirona's Community Appointment Days, including taking basic observations and providing advice and signposting. Work independently and as part of a wider team to deliver agreed priorities and objectives. Ensure all work is carried out in line with organisational values, safeguarding guidance and equality principles.

Qualifications Essential
NVQ Level 3 in Health & Social Care or willingness to work towards.
Skills, Knowledge & Experience Essential
Previous experience in a health or social care setting IT skills e.g. Microsoft Office An understanding of data protection and confidentiality Experience of working with individuals and families in a health and social care setting Access to reliable transport and able to travel independently across BNSSG
