Salary: £26,813.49 (pro rata) per annum (FTE £32,967.41)

Working hours: This post is for 30.5 hours per week, worked across the following days:

Monday: 9:00am 5:00pmWednesday: 9:00am 5:30pmThursday: 9:00am 5:00pmFriday: 9:00am 5:00pmThere may be some flexibility in daily start and finish times, and for the right candidate there may also be some flexibility regarding the non-working day. However, the total weekly hours of 30.5 will remain fixed.

Place of work: The post will be based at Acle Medical Partnership. The postholder may be required to work across other NN4 PCN practices (including Brundall Medical Partnership, Blofield Surgery, Hoveton & Wroxham Medical Centre, Staithe Surgery, Ludham & Stalham Green Surgery) in line with service need. The designated base may be subject to change in accordance with PCN service requirements.

The anticipated start date is 17 August or as soon as possible thereafter.

Main duties of the job

The post holder is a Mental Health & Wellbeing Coach who works alongside the general practice team and a mental health multi-disciplinary team within NN4 PCN. They will work within their professional boundaries to support the mental health and wellbeing of the practice population by providing wellbeing and coaching support and, where appropriately qualified and professionally registered, delivering therapeutic interventions within their own professional scope of practice.

They will also provide assistance and advice to other members of the primary healthcare team in managing the mental wellbeing of the practice population.

In NN4, some Mental Health & Wellbeing Coaches hold counselling or psychotherapy qualifications. Where this is the case, they may use those skills within their own professional scope of practice and regulatory requirements. Staff whose background is solely in health and wellbeing coaching will work within a non-clinical coaching remit.

Job description Job responsibilities

Key Responsibilities:

Work as part of a multi-disciplinary team in a first-contact patient facing role to provide personalised support to individuals, their families, and carers to empower them to manage their own wellbeing and live independently. Criteria includes; mild to moderate mental health issues such as anxiety, depression and stress, low mood/self-esteem, life events that have impacted wellbeing such as bereavement/job loss, illness etc., long term conditions impacting mental health or carers. The role does not include clinical diagnosis or case-holding; however, staff who are appropriately qualified and professionally registered may use therapeutic skills within their own professional scope of practice.Engage patients in coaching and motivate patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised wellbeing objectives.Provide interventions such as self-management education and peer support.Support personal choice and encourage steps to improve wellbeing, seeking clinical escalation where needed in line with the post-holders professional background.Work in partnership with social prescribing services to connect patients to community-based activities or support charities which enable them to take greater control of their mental health and wellbeing.Manage and prioritise a caseload of patients, taking a non-judgemental, person-centred approach that emphasises communication.Refer patients to other health professionals within the PCN when needs exceed the post-holders professional scope (e.g., GP, social prescriber, or mental health practitioner). Referrals should not include private services unless specifically requested by patients (e.g. those who have private health insurance). PCN Mental Health & Wellbeing Coaches must not charge PCN-registered patients for private services. The post-holder will escalate or refer clinical needs appropriately in line with their professional background and competence.Core competencies

Special Requirements of the Post:

Access and contribute to appropriate coaching-focussed or professional supervision proportionate to the post-holders background and scope of practice, recognising that supervision requirements for professionally registered staff are defined by their own regulatory bodies.Develop collaborative relationships and work in partnership with health, social care, community, and voluntary sector providers, as well as multidisciplinary teams, to support patients wider health and wellbeing and contribute to reducing health inequalities.Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care and behavioural approaches, and ensuring consistency in the follow-up of patients goals with MDT input.Raise awareness within the PCN of shared-decision making and decision support tools.Demonstrate understanding of safeguarding processes for vulnerable people.Implement basic safeguarding processes for vulnerable individuals.

Person Specification

Experience Essential

Minimum of 2 years experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field. An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing. Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice. An appreciation of the new NHS landscape including the relationship between individual providers, PCNs and commissioners.

Desirable

Experience of supporting individuals with mental health and wellbeing needs. Understanding of evidence-based wellbeing and behaviour change approaches. Understanding of the mentorship process.

Skills and Abilities Essential

Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face. Ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct. Excellent interpersonal, influencing, negotiation, and organisation skills, with the ability to constructively challenge views and practices in a professional and supportive manner. Ability to work under pressure and to meet deadlines. Good IT skills. Ability to maintain confidentiality. Ability to use own initiative, discretion and sensitivity.

Full driving license.

Qualifications Essential

Enrolled in, or qualified from appropriate health coaching training. Self-development through continuous professional development activity

Desirable

Willingness to undertake further study relevant to mental health in general practice supported by GP clinical supervision.
