Job summary
Bretton Park Healthcare is looking to recruit an enthusiastic and motivated Care Coordinator / Administrative Support to join our busy and friendly practice team.
This is an exciting opportunity to work within a modern GP practice environment, supporting patients to access the right care while providing excellent administrative and reception services. The successful candidate will work across both our Bretton and Park sites as part of a supportive and experienced team.

Main duties of the job
As a Care Coordinator / Administrator, you will play a key role in supporting both patients and clinicians by:
Reception and Patient Services
Welcoming patients and visitors in a professional and friendly manner. Answering telephone calls and responding to patient enquiries. Booking, amending and cancelling appointments. Processing online patient requests and enquiries. Registering new patients and updating patient records. Processing repeat prescription requests in accordance with practice procedures. Signposting patients to the most appropriate service or healthcare professional. Care Coordination
Supporting patients to navigate health and social care services. Assisting patients in managing appointments and accessing appropriate support. Liaising with clinicians, community services, care homes and external healthcare providers. Supporting vulnerable patients and those with complex health needs. Working alongside Social Prescribing Link Workers and other members of the multidisciplinary team. Following up patient actions and ensuring continuity of care where appropriate. Administrative Duties
Processing clinical correspondence and scanning documents. Managing clinician tasks and patient queries. Maintaining accurate patient records using SystmOne. Supporting the management team with administrative tasks as required.

Person Specification
Qualifications Essential
Excellent communication and interpersonal skills. Friendly, professional and patient-focused approach. Good telephone manner. Strong organisational skills and attention to detail. Ability to work effectively under pressure in a busy environment. Good keyboard and IT skills. Understanding of confidentiality and data protection. Experience working with the general public. Ability to work both independently and as part of a team.
Desirable
Previous GP practice or healthcare experience. Previous reception or customer service experience. Knowledge of SystmOne. Experience using Microsoft Office applications.
