The Diabetes and You Clinical Programme Coordinator is a clinically informed education and programme delivery role supporting the operational management, development and delivery of the Diabetes and You structured education programme across Cornwall and the Isles of Scilly.
The role combines:
structured patient education delivery,educator support and development,programme coordination,quality assurance,curriculum and resource oversight,and workforce development activity.
Main duties of the job
The post-holder will provide clinical insight and operational leadership to support safe, effective and sustainable delivery of Diabetes and You aligned to NICE structured education standards and QISMET requirements. The role will support delivery of:
patient structured education courses,educator forums and mentorship,Train the Trainer activity,programme reporting and evaluation,educator coordination,and ongoing development of educational content and resources.
The post-holder will work closely with the Education Lead, educators, Primary Care Networks (PCNs), practices and wider system partners to improve access, quality and consistency of structured diabetes education across Cornwall and the Isles of Scilly.
The role combines clinical facilitation, workforce development and programme leadership, requiring the ability to work flexibly across education delivery and operational coordination responsibilities.

Key Duties and Responsibilities
The Diabetes and You Clinical Project Lead will support the operational delivery, coordination and ongoing development of the Diabetes and You structured education programme across Cornwall and the Isles of Scilly.
The role combines programme coordination, educator support, structured patient education delivery and clinical input to support safe, effective and sustainable programme delivery aligned to NICE structured education standards and QISMET requirements.
Programme Coordination & Operational Delivery
Support day-to-day operational management of the Diabetes and You programme.Support and coordinate local delivery of Diabetes and You courses across Cornwall and the Isles of Scilly through Primary Care Networks (PCNs), practices and commissioned educators.Support planning and coordination of educator delivery, venues and programme activity.With admin support maintain oversight of programme activity, registrations, attendance and completion data.Support programme reporting, evaluation and quality monitoring requirements.Support development and implementation of innovative and sustainable operational processes to improve programme delivery and accessibilityWork collaboratively with the Education Lead and wider Training Hub team to support programme sustainability and continuous improvement.Support communication and engagement activity relating to the programme, including liaison with PCNs, practices and system partners.Identify gaps in delivery capacity and support development of solutions to maintain equitable programme access across Cornwall and the Isles of Scilly.Support exploration and development of new approaches to improve engagement, accessibility and uptake of structured education.Structured Patient Education Delivery
Deliver Diabetes and You structured education sessions to patients, carers and families where appropriate.Support and facilitate delivery capacity across PCNs and educator teams, providing direct delivery where required to support service continuity, workforce development or gaps in provision as able.Facilitate group education sessions in an engaging, inclusive and supportive manner.Promote patient self-management, behaviour change and understanding of type 2 diabetes.Deliver both face-to-face and virtual education sessions as required.Ensure delivery remains aligned to agreed curriculum, programme standards and educational principles.Support engagement and re-engagement of participants where required.Maintain sensitivity to emotional, behavioural and educational needs of participants.
Educator Support & Workforce Development
Support recruitment, onboarding and ongoing support of Diabetes and You educators.Deliver Train the Trainer sessions and educator development activity.Support delivery of educator forums, Communities of Practice and peer support activity.Provide mentorship, guidance and support to educators delivering the programme.Support workforce development aligned to structured education delivery and long-term conditions management.Contribute to ongoing review, innovation and development of educational materials, presentations and programme resources
Clinical & Quality Support
Provide clinical insight to support safe and evidence-based programme delivery.Support curriculum updates to ensure content reflects current evidence, guidance and best practice.Support quality assurance processes aligned to QISMET and NICE structured education standards.Contribute to audit, evaluation and continuous improvement activity.Escalate risks, issues or governance concerns appropriately.
Collaboration & System Working
Develop effective working relationships with Primary Care Networks (PCNs), GP practices, educators and wider system partners.Work collaboratively with specialist diabetes teams and relevant stakeholders to support integrated pathways and consistent messaging.Promote equitable access to structured education across Cornwall and the Isles of Scilly, including underserved and rural communities.Support development of flexible delivery models including virtual, community and evening delivery where appropriate.

Person Specification
Qualifications Essential
Current NMC/HPC or equivalent professional registration. Evidence of ongoing CPD and professional development.
Desirable
Teaching/education qualification, willingness to work towards. Diabetes or long-term conditions related qualification/experience. Behaviour change/coaching training.
Experience Essential
Experience working within healthcare, long-term conditions or workforce development. Knowledge of type 2 diabetes and principles of self-management support. Understanding of primary care and multidisciplinary working.
Desirable
Experience facilitating education, training or group sessions. Experience providing clinical leadership, educator support, service development or leading areas of programme delivery.
