The Role
As a key member of our clinical team, you will help us keep quality at the heart of everything we do. Your responsibilities will include:
Conducting holistic patient assessments and creating tailored care plans Delivering immunisations, wound care and health screenings Running chronic disease clinics (e.g., diabetes, asthma) Providing family planning advice and performing cervical screening Supporting minor surgical procedures Promoting health education and disease prevention Maintaining accurate patient records and documentation Contributing to audits and quality improvement initiativesIf you want to join a practice that genuinelyvalues people over process, youll feel right at home here.

Job description Job responsibilities
Deliver high-quality clinical care including chronic disease management (Asthma, COPD, Diabetes, Heart Failure, Dementia, HTN), cervical screening, contraception reviews, wound care, venepuncture, ECGs, INR management, immunisations (routine and travel), and minor illness support in line with GMS/AMS and QOF requirements.
Administer adult and childhood vaccinations in accordance with national guidelines and PGDs, ensuring competency in anaphylaxis management, resuscitation techniques, and infection prevention and control standards.
Support health promotion and lifestyle advice, undertake opportunistic screening, and contribute to disease prevention, early detection and patient education initiatives.
Maintain accurate, contemporaneous clinical records, participate in audit cycles, complete IPC audits, and contribute to quality improvement and compliance with organisational and national standards.
Work collaboratively within the multidisciplinary primary care team, liaising across primary, secondary and tertiary services, and act as an advocate for patients when required.
Provide mentorship and support to healthcare assistants, students and other learners, while maintaining continuing professional development, mandatory training, and professional registration requirements.
Manage workload and resources effectively, maintain stock and equipment, and ensure safe practice within safeguarding, legal and professional boundaries.

Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC) Membership of a professional body NMC (Nursing Midwifery Council)
Desirable
Knowledge of health promotion strategies Aware of accountability of own role Knowledge of needs of patients with long-term conditions and chronic disease Knowledge of patient group directions and associated policy Experience in Chronic Disease Management or willingness to undertake training. Clinical skills cervical cytology, immunisation and vaccination, Change-management skills and ability to support patients to change lifestyle Communication skills, both written and verbal including communicating difficult messages to patients and families Negotiation and conflict management skills IT skills Ability to work on own initiative
