The Frailty Service is built on strong multidisciplinary working, with daily consultant-led MDT discussions and close collaboration with ACPs, Hospital at Home teams, therapists and community services. This role offers the chance to make a real difference to frail older patients by delivering patient-centred care closer to home. You will support early discharge, prevent unnecessary admissions and provide high-quality step-down care, helping patients maintain independence and dignity in familiar surroundings.

Main duties of the job
The Speciality Doctor in Community Geriatric Medicine plays a key role in supporting frail patients following discharge from hospital, enabling earlier discharge and providing ongoing, high-quality care in the community.
The post holder will independently review patients, work closely with a multidisciplinary team (MDT), and contribute to service development to improve patient flow, continuity of care, and hospital-community integration.

Job description Job responsibilities
The Frailty Service is based in a purpose built three-ward block comprising three inpatient wards. The Consultant Geriatricians also have responsibility for inpatients in Leominster Community Hospital and Ledbury discharge to assess beds, undertake ward referrals from other specialties, support the Front Door Frailty team based in the Frailty Same Day Emergency Care (FSDEC) Unit, Virtual Ward and provide a general and movement disorders out patient service.

PHYSICAL REQUIREMENTS Essential
Ability to carry out duties as set out in the attached job description competently and subject to any aids and / or adaptations as may reasonably be required Completed application as outlined in the job advert Valid full driving licence and ability to travel between community sites and undertake home visits.
ATTAINMENTS AND QUALIFICATIONS Essential
Full registration with GMC licence to practise MBChB or equivalent 48 months NHS experience as a paid doctor Experience of working in Geriatric Medicine Ability to work at Specialty Doctor level evidenced by either completion of IMT2, completion of stage one equivalence or previous experience working at specialty doctor level.
