At Wye Valley NHS Trust the Colorectal/Stoma Care Nursing Team provide holistic support to patients and their families and carers on their Colorectal pathway. As part of the multi-disciplinary team the successful post holder will be instrumental in helping to develop a quality specialist Colorectal nursing service and new pathways as part of an evolving Service to meet the needs of our patients both in the acute and community settings. We are involved in all aspects of prevention, diagnosis, treatment and management of patients with colorectal disorders.

Main duties of the job

The post holder should be highly motivated and demonstrate excellent communication and interpersonal skills. They will be required to work autonomously and as part of a team and in order to cope with the demands of the role will need to be flexible and resilient and demonstrate sensitivity to others in sometimes distressing situations.

The post holder will deliver high quality and clinically effective colorectal / stoma care nursing to the populous of Herefordshire and South Powys based at Hereford County Hospital. They will act as a key worker for each individual patients for all stages of the colorectal/stoma care pathwayproviding holistic support to patients and their families and carers.

The post holder will be expected to promote education and information to raise awareness of colorectal / stoma care issues to health care professionals and the public. Incorporate research findings into clinical practice and contribute to the achievement of corporate objectives.

Job description Job responsibilities

Provide specialist Colorectal/Stoma care advice and support to patients, their families and carers and other healthcare providers throughout the patients care pathway. This will take place in both an in-patient and out-patient setting. Work as a core member of the Multidisciplinary Colorectal Team at the Wye Valley Trust contributing to meetings, audits and care pathways as required. Be an active participant in already established Nurse Led pathways and will be offered additional training in order to support this element of the role. Participate in the education and training of patients, family and carers and other health care providers within the speciality. Make use of audit and research data to evaluate the service and ensure that best evidence based care is provided. Be actively involved in health promotion and prevention strategies and adhere to Wye Valley Trust policies and procedures.
