Job summary

Aldeburgh Community Hospital has an exciting opportunity for a Band 6 Physiotherapist- 37.5 hours per week.

Aldeburgh Community Hospital is one of four East Suffolk Community rehabilitation locations, alongside Bluebird Lodge and Felixstowe Community Hospital and Avocet court care facility, as part of the wider ESNEFT Trust. The role is based at Aldeburgh with the opportunity to experience working at all four community sites.

You will work as part of the Therapy team, Including Therapy team leads, an Occupational Therapist, Therapy Assistant Practitioners and Generic Workers, to promote the discharge to assess pathways for in-patients. You will also work alongside the multidisciplinary team of Modern matron, Doctors, Nurses, Social Care workers and Ipswich Hospital transfer of care hub.

We are looking for a Physiotherapist with commitment and enthusiasm, who has a passion to work as part of a team to progress patients rehabilitation goals and use a range of therapy interventions to facilitate timely and appropriate discharges

We have ambition to include a clinical academic element into the portfolio for this post holder if desired. This may include working on portfolio studies, application to NIHR funding opportunities or developing your own homegrown research. If interested, we would welcome the successful applicant into the ESNEFT Clinical Academic Forum, which, supported by the research team, provides a network for our growing research active AHP's.

Main duties of the job

To deliver a high standard of Therapy service at Aldeburgh Community Hospital. To take part under the guidance of the Modern Matron, Therapy Lead and senior members of the team in managing bed occupancy at the Community Hospitals sites. To be responsible alongside the multi-disciplinary team for daily caseload management of therapy interventions ensuring all patients receive timely, appropriate, effective treatment and discharge planning. To work as an autonomous clinical practitioner in providing complex holistic patient-centred assessments and treatments which are governed by evidence-based practice and follow current NICE Guidelines; ensuring all clinicians involved in patient care keep their practice relevant and current whilst also adhering to HCPC, CQC and Trust clinical and quality standards. To ensure as far as is reasonably practical the safety of the patients in your care. To participate in own and others continuous professional development, appraisal processes and other developmental activities according to professional registration and trust policies. To supervise and assist in the development of junior clinicians, non-qualified staff and students on placement. To undertake audit of both clinical practice and processes to increase efficiency and effectiveness. To work with patients across Community and Acute Hospital settings as part of an Integrated Service.

Person Specification

Experience Essential

Significant post registration experience Experience of managing frailty Experience of facilitating discharges for people with complex needs and involving multi-pathology Experience of complex caseload management Documented evidence of continuing professional development involving specialist additional knowledge and experience Experience of supervision and professional/clinical development of staff Experience of demonstrating moving/handling

Facilitating training and education Project leadership Evidencing and auditing practice Participation in service development and change management Experience of participating or conducting research

Qualification Essential

Diploma/Degree/MSc pre reg State registration with HCPC Post graduate training relevant to integrated care of older people/complex discharges and involving acquisition of specialist skills and knowledge Student placement practice qualification Current UK driving license

Desirable

Membership of professional body Membership of CSP Specialist Section e.g. AGILE Accredited wheelchair assessor

Knowledge Essential

Knowledge and application of Physiotherapy assessments and interventions relevant to acute care /complex discharges Knowledge of Care pathways and whole systems approaches to discharge planning/integrated therapy intervention Knowledge and application of relevant legislation and current best practice within acute care / NICE/ National frameworks Knowledge and application of principles of clinical governance processes

Desirable

Knowledge and application of secondary prevention measures re stroke, falls etc. Knowledge and application of PDR practice and principles Knowledge of local community agencies, services, and resources Knowledge on integration of services
