We have an exciting opportunity to welcome a Clinical Nurse Specialist, to join our Community Nurse Specialist team, based in and working throughout Taunton.

You will undertake the holistic assessment, planning and delivery of specialist palliative and supportive care for patients, with advanced, progressive life limiting disease.

Regardless of whether you are a well-established clinical nurse specialist or willing to undertake a developmental clinical nurse specialist plan, we would welcome your application.

Salary: £39,920.27 - 48,070.94 per annum (pro rata if part time) Additional Payments: 30% Unsocial Hours on Saturday and 60% on Sunday & bank holidays Working Pattern: Up to 37.5 hours per week available Contract: PermanentThis role requires working weekends and bank holidays on a rota basis as well as contributing to the out-of-hours advice line; therefore, flexibility and teamwork are crucial in providing this service.

Main duties of the job

As a Clinical Nurse Specialist, you will manage a caseload of patients, delivering holistic care and specialist advice to support patients and their families through complex and sensitive stages of their journey.

You'll work closely with GPs, community teams and other professionals, acting as a key resource for palliative care across the local healthcare system.

Job description Job responsibilities

The Community Clinical Nurse Specialist (CNS) within the community team is a specialist practitioner with extensive knowledge, skills and competency to manage a caseload of patients with advanced, progressive life limiting disease.

The CNS will undertake the holistic assessment, planning and delivery of a consistently high standard of specialist palliative and supportive care in accordance with the hospice policy, procedures and standards.

The CNS will be expected to provide specialist support to people within their own homes. These visits need to be arranged based on need. New referrals into the service will be triaged by a clinician and visited within a timeframe according to our referral criteria.

The CNS will establish effective working relationships with specified Primary Health Care teams and act as a resource for professionals and non- professionals involved in palliative care within the community.

Community Nurse Specialists are expected to support the 24/7 service by providing appropriate clinical and non-clinical palliative care advice and support for patients, carers and health care professionals, within the boundaries of the service and your competence, which includes the Out of Hours Advice line and 7 day working.

Person Specification

Education and Qualifications Essential

Registered Nurse 1 (RN1)

Degree level or equivalent study in related subject, such as acute oncology, palliative care, chronic conditions management (or willing to work towards).

Post registration qualification or experience in palliative care/oncology or related subject

Recognised teaching qualification or prepared to work towards.

A willingness to undertake further study at Masters level regardless of point of entry

Commitment to continuing education and professional development at an advanced level

Desirable

Non-medical prescribing qualification

Advanced Communication skills including negotiation and diplomacy skills

Physical assessment and clinical reasoning (PACR)

Management Qualification

Computer literate, able to use word documents, database and e-mail communication.

Ability to travel to locations within the community

Desirable

Experience of community nursing Understanding and commitment to audit and research.

Excellent presentation and report writing skills.

Knowledge of End-of-Life Care tools and their application in practice.
