Job summary <h2>Job Introduction</h2><p>We have an exciting opportunity for an enthusiastic and motivated Band 4 Diabetes Associate Practitioner to join our friendly and supportive diabetes service in Swale.Working under the supervision of registered clinicians, you will play an important role in delivering structured, high-quality diabetes care across community and inpatient pathways. You will support patients living with diabetes to better understand and manage their condition, helping to improve outcomes and reduce complications.This role is ideal for someone with experience in health or social care who has a strong interest in diabetes, health promotion and long-term condition management, and who is ready to develop their clinical skills within a specialist service. You will work closely with nurses, dietitians and other healthcare professionals as part of a multidisciplinary team, contributing to patient-centred, evidence-based care.</p><h2>Package Description</h2><p>You will join a knowledgeable and welcoming diabetes team that is committed to delivering high-quality care and supporting colleagues to grow and develop. You will be encouraged to build your skills, share ideas and make a meaningful contribution to improving diabetes services for local communities.You will receive:</p><ul><li class="___ccc16d0 fje8fi8 f1ng9h0j f1bwykku f18jd3zf">A salary of £25,200.00 to £27,825.00 FTE with access to our group pens
