Job summary
The postholder will deliver high-quality patient care primarily through clinical triage, alongside remote and face-to-face consultations where appropriate. This role supports effective management of patient demand by assessing clinical needs, prioritising care, and directing patients to the most appropriate care pathway. The postholder will work flexibly across service areas in response to daily demand and workforce requirements.
This is an exciting opportunity to be part of an evolving triage service, helping to improve patient access while working as part of a multidisciplinary practice team.
There is a requirement to see extra or emergency patients.
Main duties of the job
Clinical Triage
Triage incoming patient requests predominantly via online consultation platforms (e.g. Accurx, Anima)
Undertake telephone and/or video consultations where further assessment is required
Assess patients presenting with undifferentiated and acute conditions, determining urgency and clinical need
Make safe and effective decisions based on limited or asynchronous information
Clinical Decision Making
Determine appropriate outcomes, including:
Self-care advice
GP or nurse appointments
Urgent same-day review
Referral to external services
Escalate complex or high-risk patients appropriately
Apply robust safety-netting in all patient interactions
Same-Day Face-to-Face Clinical Care
Provide face-to-face same-day clinical appointments for patients requiring in-person assessment
Undertake comprehensive patient consultations, including:
History taking
Clinical assessment and examination
Diagnosis
Treatment and management planning
Evaluation and follow-up of care
Demonstrate safe, competent clinical decision-making and deliver expert care within scope of practice
Utilise advanced assessment, diagnostic, and prescribing skills (where qualified) to manage patients within general practice
Escalate or seek senior clinical support appropriately where required
Direct patients to the most appropriate clinician or service within the practice or externally
Work collaboratively with the multidisciplinary team (GPs, pharmacists, ANPs, reception)

We welcome applications for both parttime and fulltime roles (minimum 27 hours per week).
Our ideal candidate will be able to provide both triage and facetoface clinics as part of their weekly schedule. We are also open to discussing remote working where appropriate.
We offer genuine flexibility in how triage hours are arranged including the option for shorter shifts or varied working patterns to help you balance work with life outside the clinic.

Knowledge & Skills Essential
Advanced clinical assessment and decision-making skills Ability to assess clinical risk and prioritise care with incomplete information Ability to manage both remote and face-to-face patient consultations safely and effectively Excellent communication skills (verbal and written) Strong IT/digital skills Ability to prioritise workload and work effectively in a fast-paced, high-demand environment

Understanding of primary care pathways, community services and care navigation Knowledge of safeguarding frameworks and responsibilities Prescribing competence (where qualified) and understanding of medicines management in primary care
