Job summary

Wetmore Road Surgery is looking to appoint a motivated part time Practice Nurse, to join our friendly and supportive team.

The ideal candidate will possess a Registered Nurse qualification and be experienced in management of long-term conditions, cervical cytology, childhood vaccinations and all aspects of General Practice Nursing. However, training will be provided for the right candidate. Flexibility and the ability to work as part of a team are essential.The role also includes visiting housebound patients in their homes to conduct their annual reviews, ensuring their ongoing healthcare needs are assessed and appropriately managed.

Salary is dependent upon experience and qualifications.

Main duties of the job

Responsibility for the shared diagnosis, assessment, and management of patients with long term conditions. Offer advice and guidance to individual patients and develop individual plans to promote a healthy lifestyle.

Implement and participate in vaccination and immunisation programmes for both adults and children.

Support and manage health needs of women presenting for cervical cytology screening.

Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines. Perform Doppler assessment with ABPI Messi device.

Suture removal.

Take ECGs.

Perform nebulisation.

Urinalysis

Blood glucose monitoring

Travel health risk assessment and immunisation

Assist in the provision of minor surgery steroid injection clinics and IUCD insertion.

Able to perform cardio-pulmonary resuscitation and recognise and manage anaphylaxis according to current UK guidelines.

Awareness of statutory and local child and adult safeguarding procedures, including recognition and systems of referral.

Able to chaperone and assist patients who are being examined by another clinician.

Liaise with the doctors and other primary health care professionals regarding patient care.

Recognise, assess, and refer patients with mental health needs.

Assist in the provision of mental health care reviews.

Ensure adequate stocking up and rotation of equipment in clinical rooms.

JOB TITLE: PRACTICE NURSE

Job Summary:

The post holder is responsible for the delivery of general practice nursing to the whole practice population in accordance with the NMC Code of Conduct. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative healthcare to the practice population. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures.

Job Responsibilities:

Clinical Practice:

Responsibility for the shared diagnosis, assessment, and management of patients with long term conditions. Offer advice and guidance to individual patients and develop individual plans to promote a healthy lifestyle. Carry out annual review appointment at the patient's home if they are housebound.

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome various levels of understanding, cultural background and preferred ways of communicating.

Anticipate barriers to communication and take action to improve communication.

Estimate and maintain effective communication with individuals and groups within the practice environment and with external stakeholders.

Act as an advocate when representing patients and colleagues.

Ensure effective relationships and communications with the Primary Health Care Team and other agencies.

Managerial:

Manage own administration workload and areas of responsibility as agreed with the lead nurse.

Assess plan and implement treatment programmes that promote health and wellbeing.

Manage and organise individual patient consultations. Produce accurate complete records of consultations according to NMC guidelines and obtain and document informed consent (either verbal or written).

Deliver care according to Nice guidelines and evidence-based care.

Attend and participate in relevant staff meetings.

Participate in the maintenance of quality governance systems and processes across the practice.

Identify changes to clinical practice that are required to implement evidence-based guidelines.

Participate in audit where appropriate.

Educational:

Identify personal development and training needs in conjunction with the Lead Nurse at annual appraisal.

Participate in clinical supervision.

Support others with their training and development needs.

To participate in continuing education and complete mandatory training to maintain level of professional knowledge and skills.

Participate in the support, education, and assessment of students within the practice.

Team Working:

Understand own role and scope and identify how this may develop over time.

Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
