Diabetes Specialist Nurse

We are looking for an experienced Diabetes Specialist Nurse who is an Independent Prescriber to join our growing clinical team. This role will support adults living with diabetes through a remote-first, structured support service.
You will help patients understand their glucose patterns, build confidence in self-management, use technology safely and make informed decisions about their diabetes care. You will work within a supportive, consultant-led service alongside colleagues across Spirit Health, GP practices and wider NHS teams.

Review glucose data and wider clinical information.
Provide clear, personalised diabetes education and advice.
Support safe medicines optimisation and prescribing within your scope of practice.
Identify and escalate clinical concerns appropriately.
Maintain accurate clinical records and communicate clearly with GP practices.
Contribute to a safe, compassionate and practical service for patients.

Person Specification
Experience Essential
Confidence supporting people with Type 2 diabetes. Experience with insulin management and diabetes medication optimisation.
Desirable
Excellent communication skills, particularly when working remotely. Sound clinical judgement and strong documentation standards. A warm, patient-centred approach.
Qualifications Essential
Current NMC registration Independent prescribing qualification Strong diabetes experience, ideally as a Diabetes Specialist Nurse or in a diabetes-focused role in a community setting.
Desirable
Confidence supporting people with Type 2 diabetes. Experience with insulin management and diabetes medication optimisation. Excellent communication skills, particularly when working remotely.
