Main duties of the job The Team Leader role at Avery Healthcare involves supporting the home management team to deliver high-quality care. This role requires creating a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Responsibilities include leading person-centred care planning processes, assisting with transitions of residents, monitoring well-being, and practicing safe systems of work. Candidates will live by Avery's values of caring, supportive, honest, respectful, and accountable behavior. The job requires an NVQ 3 or equivalent in care for care homes and two years of experience in a senior position with the resident group. The ability to communicate effectively, lead staff and act responsibly in the manager's absence is essential. A DBS check is required, which Avery Healthcare will cover, along with proof of eligibility to work in the UK.

ABOUT THE ROLE

Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Other responsibilities will include:

Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans. Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate. Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation. Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

Our ideal candidate must:

Have anNVQ 3 or equivalent in care for care homes. Two years experience working in a senior position with the resident group. Have the ability to communicate effectively both verbally and in writing. Demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Person Specification

Qualifications Essential

NVQ 3 or equivalent in care for care homes, two years experience working in a senior position with the resident group, effective communication skills, leadership capabilities.
